Since the introduction of fractional flow reserve (FFR) [1], coronary physiology has progressively gained prominence in coronary diagnosis and intervention. Its implementation has led to improved outcomes, safely avoiding unnecessary revascularisations without increasing or even decreasing the number of adverse events. Recently, FFR has facilitated a new approach to understanding ischemic heart disease, explaining its symptoms and consequences beyond coronary stenoses [2].
