Impact of care coordination on 30-day readmission, mortality, and costs for heart failure
CONCLUSIONS: High care coordination is associated with decreased 30-day readmission, mortality, and costs for HF. Enhancing coordination of care has the potential to increase the value of care. It is important to monitor coordination of care and develop strategies to maintain high levels of care coordination for HF.PMID:38603537 | DOI:10.37765/ajmc.2024.89533
