One-Lung Ventilation in a Small Child

We congratulate Maddali et al. for the successful management of a challenging case involving a 2-yr-old undergoing one-lung ventilation for right thoracotomy.1 Withdrawal of a single-lumen endotracheal tube from the left mainstem bronchus is also helpful to temporarily resume bilateral ventilation when oxygen desaturation occurs during one-lung ventilation. Placement of a single-lumen endotracheal tube into the left mainstem bronchus may be difficult and can be facilitated by rotation of the bevel (depending on the model) and patient's head before advancement.
