Estimating the Proportion of Heart Failure Admissions Potentially Eligible for Hospital at Home

Hospital at home (HaH) is an emerging care model where patients requiring hospitalization receive inpatient-level care at home. Services include twice daily nursing visits with laboratory collection, intravenous medications, patient education, and other acute care services. Current regulations also require daily physician or advanced practice provider visits and other hospital services such as radiology and physical therapy. Programs sometimes require patients to have a certain level of independence or a caregiver to provide support between nursing visits.
