Personalized Pacing for HFpEF: myPACE

The misperception that pharmacologic heart rate lowering is beneficial in heart failure with preserved ejection fraction (HFpEF) has contributed to an overuse of beta-blockers. It is commonly argued that a slowing of heart rate provides more time for diastolic filling – However, this statement ignores that prolonged diastolic filling time can only increase ventricular filling volume (i.e., preload) if filling pressures increase, particularly if filling is restricted by a stiff left ventricle (LV) as shown in the Figure.
