Optimization Of Guideline Directed Medical Therapy Utilizing Standardized Electronic Documentation During Heart Failure Hospitalization

Heart failure is a progressive disease that greatly affects quality of life. This disease is associated with a high 5-year mortality rate, high frequency of hospitalizations and functional limitations. The use of guideline directed medical therapy (GDMT) has been shown to reduce hospitalizations and greatly improve functional status. However, adherence to GDMT and optimization is challenging. We implemented a standardized electronic medical record (EMR) checklist to assure adherence. We conducted a retrospective study with the aim to evaluate the impact of standardized documentation EMR on GDMT adherence.
