Improving Education On Discharge For Patients Admitted With Heart Failure
Heart failure (HF) remains a leading cause of hospitalization among US adults. Studies have shown that education decreases readmission rates and improves outcomes. The American Heart Association (AHA) created an educational booklet describing symptoms, treatments, and eating plans for heart failure. The goal of this project was to increase utilization of the AHA workbook in the form of discharge instructions to provide HF patients with valuable information.
