Tuberculous peritonitis

A 62-year-old man presented with a 2-month history of abdominal swelling, loss of appetite, and a significant 5-kilogram weight loss. Physical examination revealed emaciation, abdominal swelling, and shifting dullness. Lab tests showed monocytosis, elevated erythrocyte sedimentation rate and C-reactive protein. An 18F-fluorodeoxyglucose (18F-FDG) PET/CT scan found lung calcification, pleural effusion, mesentery, and peritoneum thickening with diffuse 18F-FDG accumulation (Fig.  1). Tuberculosis tests including anti-tuberculosis antibody test and tuberculin skin test were negative except for a slightly positive tuberculosis spot test.
