Exploring and Predicting Heart Failure Risk and Progression Using Electronic Medical Records

Improving outcomes in the heart failure epidemic requires a better understanding of heart failure hospitalisation and progression of the syndrome. Readmission and death are highly prevalent in the hospitalised heart failure patient population, with 30-day readmission rates of around 20%, over half of the patients readmitted within a year, and an estimated 5-year mortality rate of 75%.
