Promethazine/codeine on top of Percocet and diazepam?
Just wondering. Is there ever a good reason to give promethazine with codeine for cough to someone who is already on Percocet and diazepam? The prescriber insisted that we fill the promethazine/codeine. (Source: Student Doctor Network)

What PRN medication would you prescribe for agitation in outpatient setting?
The patient is a young adult with unspecified mental handicap who comes with mom. He has been on the following regimen for years: clonidine 0.3mg per day depakote DR 1000mg per day geodon 200mg per day valium 20mg per day (Most of the above medications are split into two dosings a day.) He has been on the regimen for years and is tolerating medications. Mother does not want to change the treatment. CIII Filling (Specifically Suboxone)
A patient on Suboxone, prescribed by a Pain Management Specialist with a Buprenorphine Waiver, who doesnt fall into the normal box and has a complex medical history, meaning quitting all other substances with dependency potential is not an option, has landed in a difficult situation. The MD was prescribing Suboxone and Percocet (Yes I know, that is counterintuitive, but that's not the focus of this question). The worst polypharmacy
I may have a winner for worst polypharm ever. Young (early 20's) individual comes in after being hospitalized several times out of state after being hospitalized multiple times for anxiety and "ADD." He brought in his discharge paperwork that had his d/c meds, which included, Get this -- 2 Atypical AP's (I bet you can guess what one of them is) Strattera Adderall An SNRI Xanax Restoril Valium, and Tramadol "for pain" -- without any actual physical health conditions. Doctor's self Rx. your state's policy & your educated stance
So I am currently a resident physician and read an article discussing about self prescribing among residents. I was talking among colleagues and many of them admit to self prescribing or having a friend or wife/ husband write them an Rx. Several even admit to having prescribed class IV medications for themselves (valium, vicodin, ambien) were some of the examples. Pain after laminotomy lead
Morbidly obese 59 yo patient s/p lumbar 4-5 and L5-S1 discectomy followed by two failed PLIF. Never fused. Was off of opioids prior to SCS trial. Had successful trial and was implanted with thoracic paddle lead by experienced neurosurgeon. Complaining of burning sensation and intermittent intense spasm in area of implant (paddle) since surgery. Surgeon prescribes hydromorphone and valium.
