Incidental Anaplastic Thyroid Carcinoma
Dear colleagues, I need your opinion concerning Mypatient; 66 yo male patient , PS0, no major comprbidities, incidentally diagnosed of a small anaplastic thyroid carcinoma, following a partial thyroïdectomy for a multi nodules goiter. Ethics Violation and Medical School Application

Bit of a rant...anyone have a similar experience?
I work (academic subspecialist) with a malignant narcissist who has effectively trained our oncologists that any pathologist who doesn't order at least IHC on a given case is incompetent (slight exaggeration, but not too far off the mark). Generally this hasn't effected me too much...usually can fend off stupid requests for IHC from clinicians ("can you please run ER/PR on this papillary thyroid carcinoma?"). Radiation Recommendations?
New In-coming fellow I wanted to know what recommendations that experienced physicians can give to minimize risk. I have a heavy cancer history in my family and would like to know your recommendations in addition to the obvious lead gowns, thyroid shield, goggles, and possibly lead cap. T4a Larynx cancer with negative nodal dissection no concerning pathologic features...
s/p total laryngectomy with negative unilateral neck dissection, some invasion into the contralateral larynx and erosion through thyroid cartilage. ENT not concerned about stomal margins etc. No PNI, no LVSI. Referred him to me because that's what we do, and T4 tumors don't present this way commonly enough to really say much about forgoing adjuvant RT as far as I can tell, though the NCCN seems to allow for observation in a similar setting. Thyroid Neck Recurrence
I got a guy that presented 2 years ago with metastatic thyroid cancer; they did a hemithyroidectomy at presentation to get more definitive diagnosis. Turned out to be hurthle cell ca. They did adjuvant RAI after this although the tumor wasn't hot for iodine uptake. There are 4 mets in the lungs that is it, they are about 1-2 cm and one is 2-3 cm. RT for anaplastic thyroid carcinoma
Dear colleagues, I have an 85 yo, very gd PS that has non-metastatic thyroid anaplastic carcinoma, that has undergone debulking surgery and tracheostomy. He was referred for RT. The tumor board at his institution refused to do RT because in their experience, all pts progressed during the treatment. The pt was referred to me for second opinion. What do you recommend ?
