DIC in septic shock
I have seen a couple of patients recently with profound septic shock ; one from meningococcemia and one from E. coli sepsis, develop DIC and then lose all 4 extremities. I was thinking what would be options for combating the microvascular thrombosis ? Xigris used to be available 12 years back but was taken off the market because of disappointing data in the PROWESS trial. Was it because Xigris was being used too indiscriminately ? Meningioma and the SSS
64 yo man history of psychiatric issues with increasing forgetfulness over the last year had an MRI that showed left parasag vertex extraaxial mass (picture attached). Was taken for surgery With path showing atypical meningioma, low Ki67 but high mitoses. Considered subtotal due to wrapping around and invasion of the sup sag sinus. Surgeon was worried about RT hastening CVST (cerebral venous sinus thrombosis) and wanted to know if we would recommend RT. Don’t see many of these, is concern...
