Stroke Code Simulation Lab; Save Time Save Brain! (P2.375)
CONCLUSIONS: The stroke code simulation lab met its objectives of improving the neurology residents’ knowledge and experience managing stroke codes. It was perceived as a valuable exercise. Further data will determine if the stroke code simulation lab improves patient care.Disclosure: Dr. Mao has nothing to disclose. Dr. Zidan has nothing to disclose. Dr. Afzal has nothing to disclose. Drip-and-Ship Thrombolytic Therapy Supported by the Telestroke System for Acute Ischemic Stroke Patients Living in Medically Under-served Areas.
Authors: Kageji T, Obata F, Oka H, Kanematsu Y, Tabata R, Tani K, Bando H, Nagahiro S Abstract There are a few stroke specialists in medically under-served areas in Japan. Consequently, in remote area patients may not receive thrombolysis with intravenous recombinant tissue plasminogen activator (iv rt-PA), the standard treatment for acute ischemic stroke. Using a mobile telestroke support system (TSS) that accesses the internet via a smart phone, we implemented iv rt-PA infusion therapy under a drip-and-ship protocol to treat the stroke patients in medically under-served areas. The physicians at the Tokushima Pref...

Identifying Barriers in Acute Stroke Therapy in Argentina. ARENAS Registry (P2.014)
CONCLUSIONS: Only 47% of patients were admitted to an Intensive Care Unit, rt-PA was underused and initial evaluation was not performed by a neurologist in most of cases. The gap between clinical practice guidelines and real world is wide in Argentina and interventions to improve acute stroke care are mandatory.Disclosure: Dr. Atallah has nothing to disclose. Telemedicine remote controlled stroke evaluation and treatment, the experience of radiographers, paramedics and junior doctors in a novel rural stroke management team
CONCLUSIONS: The telemedicine-based, remote controlled, stroke evaluation and treatment was experienced, by the participants, to be well organised and of high quality. Communication and image reading appear to be the salient challenges. Regular training sessions and follow-up, as well as an evaluation of incidents by the project manager, proved to be of great importance in retaining and securing the continued running of the service and ensuring high-quality treatment. Further research is indicated in the comparison of this telemedicine service with stroke treatment given in a mainstream hospital.PMID:34090447 | DOI:10.1186/s12913-021-06591-1

Intravenous thrombolysis in ischemic stroke with unknown onset using CT perfusion
ConclusionDelayed stroke patients with unknown onset time were no different than patients >4.5 h regarding eligibility and response to CTP‐based i.v. thrombolysis.

Routine serum C‐reactive protein and stroke outcome after intravenous thrombolysis
ConclusionsAccording to our findings, elevated routine serum CRP measured within 24 h after admission does not seem to independently affect the outcome in patients receiving intravenous thrombolysis for stroke. However, further studies of blood samples taken directly before the treatment are needed.

Improvement in Rates of Thrombolytic Therapy in Acute Stroke by a Telestroke Program in Rural Northern Wisconsin (P1.011)
Conclusion: Following development of a Telestroke program in northern Wisconsin, rural stroke patients benefited from dramatic increases in access to neurological expertise and acute thrombolytic treatment. Patients and providers have embraced the program with plans to expand Telestroke to other remote access hospitals.Disclosure: Dr. Kartje has nothing to disclose. Dr. Klemm has nothing to disclose. Assessment of acute stroke cerebral CT examinations by anaesthesiologists
ConclusionsThis study suggests that anaesthesiologists who are experienced in pre‐hospital care may be quickly trained to assess cerebral CT examinations in acute stroke patients with regard to radiological contraindications for thrombolytic therapy.

Stroke of a cardiac myxoma origin
Conclusion: Cardiac myxoma stroke is rare. Often does it affect young females. For an improved diagnostic accuracy, magnetic resonance imaging of the brain and echocardiography are imperative for young stroke patients in identifying the cerebral infarct and determining the stroke of a cardiac origin. Immediate thrombolytic therapy may completely resolve the cerebral stroke and improve the neurologic function of the patients. An early surgical resection of cardiac myxoma is recommended in patients with not large territory cerebral infarct. Susceptibility-diffusion mismatch in middle cerebral artery territory acute ischemic stroke: clinical and imaging implications.
CONCLUSION: The presence of SWI-DWI mismatch in acute MCA territory ischemic infarct is associated with smaller infarct volume. Moreover, SWI-DWI mismatch was associated with better outcome after correction for infarct size, severity of admission symptoms, and age. PMID: 27799573 [PubMed - as supplied by publisher]

Acute ischemic stroke management in Lebanon: obstacles and solutions.
This study assesses the current practices of physicians in Lebanon routinely involved in ischemic stroke (IS) management. We conducted a prospective observational study of patients hospitalized at 8 different Lebanese hospitals in the period August 1, 2015 to July 31, 2016, with a diagnosis of acute stroke. Baseline characteristics and data on diagnostic studies, as well as treatments received during hospitalization and at discharge, were collected and analyzed. Two hundred and three strokes/transient ischemic attacks (TIAs) were recorded but only 173 patients (85%) with ischemic events were included in the study. The pati...

PET Detection of Cerebral Necrosis Using an Infarct-Avid Agent 2-Deoxy-2- 18 FFluoro- d -Glucaric Acid (FGA) in a Mouse Model of the Brain Stroke
ConclusionsAccumulation of FGA correlated well with the perfusion defect and inflammatory injury. As a PET agent, FGA has potential to image infarcted core in the brain stroke injury with high sensitivity, resolution, and specificity.
