Benchmarking Outcomes of Surgical Aortic Valve Replacement in Patients with Bicuspid Aortic Valves
CONCLUSIONS: In this nationally-representative study, 30-day mortality was similar but risk-adjusted 5-year mortality was significantly lower in bicuspid patients undergoing isolated SAVR compared to tricuspid patients, specifically low-risk and normal left ventricular ejection fraction patients. Long Term Outcomes and Anticoagulation in Mitral Valve Surgery - A Report from the Society of Thoracic Surgeons Database
CONCLUSIONS: Anticoagulation was used in less than half of mitral valve surgery. In MVrep patients, warfarin was associated with increased bleeding and was not protective against either stroke or mortality. In BMVR patients, warfarin was associated with a modest survival benefit, increased bleeding and equivalent stroke risk. Surgical Ablation of Atrial Fibrillation is Associated with Improved Survival Compared to Appendage Obliteration Alone: An Analysis of 100,000 Medicare Beneficiaries
Societal guidelines support the concomitant surgical ablation (SA) of atrial fibrillation (AF) in patients undergoing cardiac surgery. Recent evidence has highlighted the stroke reduction of left atrial appendage obliteration (LAAO) with or without SA in similar populations. To inform clinical decision-making, we evaluated real-world outcomes of patients with AF undergoing cardiac surgery by comparing no AF management vs. LAAO alone vs. SA+LAAO.

Reply: Stentless versus stented bioprosthetic root replacement in the medicare population and the wisdom of crowds
In our recent analysis of long-term outcomes of aortic root operations in the United States among Medicare beneficiaries using the Society of Thoracic Surgeons Adult Cardiac Surgery Database (STS-ACSD) and Centers for Medicare and Medicaid Services linked data for patients aged 65 years or older undergoing elective aortic root surgery, we concluded that the bioprosthetic Bentall operation should be the procedure of choice in this population due to improved survival and lower stroke risk compared with the use of the mechanical Bentall, and lower risk of aortic valve reintervention compared with valve-sparing aortic root replacement.

Transcatheter vs Surgical Aortic Valve Replacement in Pure Native Aortic Regurgitation
CONCLUSIONS: In Medicare patients with pure native AR, TAVR with the current commercially available transcatheter valves has comparable short-term outcomes. Outcomes of stroke events during transcatheter aortic valve implantation
CONCLUSIONS: Among Medicare beneficiaries undergoing TAVI, procedure-related ischaemic stroke was associated with worse outcomes, increased Medicare expenditures, and less time spent at home. Procedure-related ischaemic stroke during TAVI remains a critically important and potentially preventable source of patient mortality, morbidity and healthcare utilisation.PMID:35135749 | DOI:10.4244/EIJ-D-21-00951
