Result of 48-Hour Approach to Symptomatic Carotid Lesions in Patients With Stable Neurologic Examinations
Timing of treatment for symptomatic carotid disease has a controversial history with current guidelines recommending carotid surgery within 48  hours up to 2 weeks. Advocating for earlier carotid endarterectomy (CEA) after stroke (cerebrovascular accident [CVA]) or while a patient is having transient ischemic attacks (TIA) can potentially provide additional benefit to acute stroke patients resulting in improved stroke and death outcomes.

Acute ischemic stroke: The role of emergency carotid endarterectomy in isolated extracranial internal carotid artery occlusion
CONCLUSIONS: Emergency CEA in isolated eICA occlusion has proved to be a safe and effective treatment option in selected patients. CT perfusion, imaging the ischemic penumbra and quantifying the tissue suitable for reperfusion, offers a valid support in the diagnostic-therapeutic workup. Indeed, we can infer that the area of the ischemic penumbra is directly proportional to the margin of clinical improvement after revascularization, supposing that the appropriate intervention timing is respect.PMID:37594376 | DOI:10.1177/17085381231192712

Bypass and other modified reconstruction techniques for 'challenging' carotid cases: A comparison with conventional endarterectomy
CONCLUSIONS: Patients who underwent ICA revascularization with MCAR showed risks of perioperative death, major or minor stroke (<2%), reintervention rates and carotid restenosis rates that are comparable with PCEA or ECEA groups. Nevertheless, the MCAR group showed a significantly higher rate of global central neurological complications (considering together TIA, minor stroke and major stroke) than patients treated with standard CEA. The Impact of Full Collapse on the Risk of Ischaemic Stroke in Patients with Carotid Near Occlusion
Carotid near occlusion (CNO) is a severe carotid stenosis associated with a reduction in the calibre of the extracranial internal carotid artery distal to the stenosis.1 The risk of ipsilateral stroke in patients with CNO is a controversial issue. While a post hoc analysis of the North American Symptomatic Carotid Endarterectomy Trial (NASCET) and the European Carotid Surgery Trial (ECST) reported a lower risk of ipsilateral stroke for medically treated patients with CNO compared with patients with severe (70 –99%) carotid stenosis2, more recent studies have shown that the CNO recurrence rate may be higher.
