Successful heart transplantation in a patient with Loeys-Dietz Syndrome.
Abstract Our patient is a 45-year-old woman with a history of Loeys-Dietz syndrome (LDS), vertebral artery dissection, stroke, prior atrial septal defect (ASD) repair, and spontaneous left main coronary artery dissection re-vascularized with coronary artery bypass grafting who progressed to end-stage heart failure. She was listed for cardiac transplantation and a suitable organ became available. She underwent heart transplantation over one year ago successfully. This is the first reported case of a successful heart transplantation in an individual with Loeys-Dietz syndrome. PMID: 30513314 [PubMed - as supplied by publisher]

Does Mitral Valve Calcium In Patients Undergoing Mitral Valve Replacement Portend Worse Survival?
CONCLUSIONS: Conservative approach to treat MAC achieves satisfactory results. Patients with MAC have significant comorbidities contributing to a worse survival though MAC in itself is not a risk factor for mortality. PMID: 30391339 [PubMed - as supplied by publisher]

Bilateral Internal Mammary Artery Utilization in Diabetics: Friend or Foe?
CONCLUSIONS: In this statewide analysis, diabetics who received BIMA grafts (compared to diabetics with LIMA grafts or non-diabetics with BIMA grafts) had higher O/E ratios for composite morbidity/mortality as a result of higher O/E ratios for major complications. Hemiarch Reconstruction Vs Clamped Aortic Anastomosis for Concomitant Ascending Aortic Aneurysm.
CONCLUSIONS: Hemiarch replacement using DHCA with RCP does not increase the risk of operative complications compared to a normothermic, clamped-distal aortic anastomosis, and therefore its use should not be limited when planning complex multi-procedural reconstructions during elective ascending thoracic aortic replacement with concomitant cardiac surgery. The Society of Thoracic Surgeons 2018 Adult Cardiac Surgery Risk Models: Part 1 - Background, Design Considerations, and Model Development.
CONCLUSIONS: Completely new STS ACSD risk models have been developed based on contemporary patient data; their performance is superior to that of previous STS ACSD models. PMID: 29577925 [PubMed - as supplied by publisher]

The Society of Thoracic Surgeons 2018 Adult Cardiac Surgery Risk Models: Part 2 - Statistical Methods and Results.
CONCLUSIONS: New STS ACSD risk models have generally excellent calibration and discrimination and are well suited for risk adjustment of STS performance metrics. PMID: 29577924 [PubMed - as supplied by publisher]

Hybrid Coronary Revascularization Versus On-Pump Coronary Artery Bypass Grafting.
CONCLUSIONS: For selected patients, HCR is associated with a faster postoperative recovery as well as similar short-term and long-term outcomes when compared with standard on-pump CABG. PMID: 29571730 [PubMed - as supplied by publisher]

Coronary Revascularization with Single Versus Bilateral Mammary Arteries: Is It Time to Change?
CONCLUSIONS: In a large regional experience BIMA is associated with no upfront risk of adverse events and improved long-term survival compared to SIMA. Our results indicate that BIMA conduits should be considered more frequently during CABG due to their demonstrated survival advantage. PMID: 29550203 [PubMed - as supplied by publisher]

Bilateral Internal Thoracic Artery Grafts in Hemodialysis: A Single-Center Propensity Score Analysis.
CONCLUSIONS: CABG using BITA grafts showed no advantages in midterm outcome among hemodialysis patients; however, there were no adverse effects on perioperative morbidity or death. For hemodialysis patients with limited available conduits, BITA may be an important option for multivessel revascularization. PMID: 28964409 [PubMed - indexed for MEDLINE]

Should We Ablate Atrial Fibrillation During Coronary Artery Bypass Grafting and Aortic Valve Replacement?
CONCLUSIONS: Concomitant AF ablation in patients with AF undergoing CABG or AVR or both does not increase perioperative rates of mortality or morbidity. Moreover, concomitant AF ablation is effective at reducing postoperative AF burden and increases mid-term AF-free survival. PMID: 28262298 [PubMed - as supplied by publisher]

Transesophageal Echocardiography Complications in Adult Cardiac Surgery: A Retrospective Cohort Study.
CONCLUSIONS: The overall incidence of TEE complications after cardiac surgery was 1.4%. Advanced age, low body mass index, complexity of procedure, prior stroke, prolonged bypass time, and return to the operating room appear to be significant risk factors for TEE complications.
