Failure to Rescue: A New Society of Thoracic Surgeons Quality Metric for Cardiac Surgery
CONCLUSIONS: A new risk-adjusted FTR metric has been developed which complements existing STS performance measures. The metric specifically assesses institutional effectiveness of postoperative care, allowing hospitals to target quality improvement efforts.PMID:34242640 | DOI:10.1016/j.athoracsur.2021.06.025

Neurological complications in high-risk patients undergoing coronary artery bypass surgery
CONCLUSIONS: In patients with increased risk of perioperative stroke, aortic manipulation including the use of cardiopulmonary bypass or partial clamping for central anastomoses is associated with higher rates of postoperative neurological complications. These patients may benefit from off-pump surgery without aortic manipulation if complete revascularization can be ensured.PMID:34087237 | DOI:10.1016/j.athoracsur.2021.05.018

Society of Thoracic Surgeons 2021 Adult Cardiac Surgery Risk Models for Multiple Valve Operations
CONCLUSIONS: New STS-ACSD risk models have been developed for multiple valve +/- CABG operations, and these models will be used in subsequent STS performance metrics.PMID:33844993 | DOI:10.1016/j.athoracsur.2021.03.089

Ticagrelor or aspirin after coronary artery bypass in patients with chronic kidney disease
CONCLUSIONS: In patients with CKD and CABG, those who received ticagrelor had a higher incidence of MACCE but a similar incidence of major bleeding compared to those who received aspirin.PMID:33819474 | DOI:10.1016/j.athoracsur.2021.03.061

New Proximal Anastomosis Technique for Calcified Ascending Aorta in Coronary Artery Bypass Grafting
Ann Thorac Surg. 2021 Mar 6:S0003-4975(21)00442-2. doi: 10.1016/j.athoracsur.2021.01.076. Online ahead of print.ABSTRACTTo deal with calcified ascending aorta during CABG, we describe an alternative technique to create a clampless proximal anastomosis using a Foley catheter and polypropylene suture. In 30 patients, the number of distal anastomoses averaged 3.1 ± 0.7, and mean time of proximal anastomosis was 18.9 ± 1.3 minutes, respectively. Neither early nor late death occurred. Stroke occurred in 2 high-risk patients. At mean 1.6 ± 0.5 years of follow-up, 1 patient sustained recurrent angina, and graft patency was 93%...

Prosthesis selection for aortic valve replacement with concomitant coronary artery bypass graft
CONCLUSIONS: Long-term survival, overall reoperation, or stroke incidence was comparable among the two groups, while patients with a mechanical valve showed a greater likelihood of major bleeding events. Regarding the limited durability of bioprostheses, a larger sample size followed for 15 or more years will be necessary to determine the optimal aortic valve prosthesis for patients aged 50 to 69 years undergoing concurrent AVR and CABG.PMID:33667456 | DOI:10.1016/j.athoracsur.2021.02.033

Sex-based outcomes after coronary artery bypass grafting.
CONCLUSIONS: Sex was not an independent predictor of long-term MACE after CABG in this population-based cohort study. Men had however higher long-term all-cause mortality and women higher risk of myocardial infarction. Long-term outcomes should be accounted for when considering sex as a risk factor for CABG. Initial Clinical Experience with Mitral Valve Translocation for Secondary Mitral Regurgitation.
CONCLUSIONS: Mitral valve translocation creates a large surface of coaptation and effectively corrects FMR. Further study is needed to demonstrate the long-term durability and clinical utility of this operation. PMID: 33440174 [PubMed - as supplied by publisher]

Aortic Annular Enlargement in the Elderly: Short and Long-Term Outcomes in the United States.
CONCLUSIONS: These data suggest annular enlargement during SAVR is associated with increased short-term risk in a Medicare population. Survival curves crossed after three years, which may portend a benefit in select patients. However, annular enlargement is still only done in the minority of patients who are at risk for PPM. Hyponatremia: an overlooked risk factor associated with adverse outcomes after cardiac surgery.
CONCLUSIONS: Preoperative hyponatremia is relatively common and is associated with adverse short- and long-term outcomes after cardiac surgery. Preoperative hyponatremia can be used independently from standard risk factors to identify high risk patients for cardiac surgery. PMID: 33080237 [PubMed - as supplied by publisher]
