Incidence of Postoperative Stroke Using the Heartstring Device in 1,380 Coronary Artery Bypass Graft Patients With Mild to Severe Atherosclerosis of the Ascending Aorta.
CONCLUSIONS: Compared with the Society of Thoracic Surgeons predicted risk for stroke, the Heartstring proximal anastomotic device can be safely used with all aortic grades. The most prominent benefit appears to be for patients with grade II disease and greater. PMID: 24726605 [PubMed - as supplied by publisher]

Epiaortic Ultrasound to Prevent Stroke in Coronary Artery Bypass Grafting.
CONCLUSIONS: Avoiding aortic manipulation is associated with the lowest risk of stroke in patients undergoing CABG. When manipulation of the ascending aorta is planned, EAU is effective in guiding the surgical strategy to reduce the risk for embolic stroke in these patients. Off-Pump Coronary Artery Bypass Reduces Early Stroke in Octogenarians: A Meta-Analysis of 18,000 Patients.
CONCLUSIONS: Coronary artery bypass in octogenarians can be performed safely with low early mortality. Although off-pump operations reduce the risk of early stroke, all other adverse events are comparable in on- and off-pump coronary artery bypass operations. Data regarding late mortality is at present limited; however, both on- and off-pump procedures appear to produce comparable survival. PMID: 25791924 [PubMed - as supplied by publisher]

Institutional Variation in Mortality After Stroke After Cardiac Surgery: An Opportunity for Improvement.
CONCLUSIONS: Institutional variation, more so than individual patient risk factors, is highly associated with postoperative stroke and FTR rates after stroke after cardiac surgery. Postoperative stroke remains significantly associated with mortality and morbidity. Institutional practice patterns may confer a disproportionate influence on postoperative stroke independent of case mix. Understanding differences between high and low performing centers is essential to improving outcomes, costs, and hospital quality. PMID: 26188969 [PubMed - as supplied by publisher]

Meta-Analysis of Staged Versus Combined Carotid Endarterectomy and Coronary Artery Bypass Grafting.
CONCLUSIONS: Our meta-analysis of observational studies suggests comparable outcomes in combined and staged approach for synchronous carotid and coronary artery disease. Hence, the 2 strategies can be used interchangeable in the clinical practice, with each having specific applications linked to specific clinical conditions. A randomized trial is warranted to answer this question definitively. PMID: 24090581 [PubMed - as supplied by publisher]

Neurological complications in high-risk patients undergoing coronary artery bypass surgery
CONCLUSIONS: In patients with increased risk of perioperative stroke, aortic manipulation including the use of cardiopulmonary bypass or partial clamping for central anastomoses is associated with higher rates of postoperative neurological complications. These patients may benefit from off-pump surgery without aortic manipulation if complete revascularization can be ensured.PMID:34087237 | DOI:10.1016/j.athoracsur.2021.05.018

Bilateral Internal Mammary Artery Utilization in Diabetics: Friend or Foe?
CONCLUSIONS: In this statewide analysis, diabetics who received BIMA grafts (compared to diabetics with LIMA grafts or non-diabetics with BIMA grafts) had higher O/E ratios for composite morbidity/mortality as a result of higher O/E ratios for major complications. Prosthesis selection for aortic valve replacement with concomitant coronary artery bypass graft
CONCLUSIONS: Long-term survival, overall reoperation, or stroke incidence was comparable among the two groups, while patients with a mechanical valve showed a greater likelihood of major bleeding events. Regarding the limited durability of bioprostheses, a larger sample size followed for 15 or more years will be necessary to determine the optimal aortic valve prosthesis for patients aged 50 to 69 years undergoing concurrent AVR and CABG.PMID:33667456 | DOI:10.1016/j.athoracsur.2021.02.033

Gender Differences in Failure-to-Rescue After Coronary Artery Bypass Grafting
CONCLUSIONS: Women who underwent isolated, non-emergent CABG had statistically similar frequencies of FTR compared to their male counterparts despite experiencing greater rates of morbidity and mortality. Further efforts to narrow the gender outcome gap after CABG should focus on preoperative and intraoperative phases of care instead of postoperative management.PMID:34774815 | DOI:10.1016/j.athoracsur.2021.09.070

Off-Pump and On-Pump Coronary Revascularization in Patients With Low Ejection Fraction: A Report From The Society of Thoracic Surgeons National Database.
CONCLUSIONS: In The Society of Thoracic Surgeons National Database, OPCAB is associated with significantly reduced adjusted risk of early morbidity and mortality for patients having coronary bypass grafting with an ejection fraction of less than 0.30. Transapical Aortic Valve Implantation in Patients With Previous Cardiac Surgery.
CONCLUSIONS: Transapical transcatheter aortic valve implantation has simplified surgical treatment of high-risk patients with previous cardiac surgery and severe aortic valve stenosis and is associated with minimal risk of stroke. Furthermore, current data suggest that the presence of previous cardiac surgery does not impair outcomes after transapical transcatheter aortic valve implantation, making this subset of patients particularly applicable for this evolving approach. Midterm Outcome of Coronary Artery Bypass Grafting in Young Patients: A Multicenter Italian Study.
CONCLUSIONS: Patients less than 50 years of age undergoing CABG have an excellent outcome compared with elderly patients. These data indicate that despite its premature onset, coronary artery disease in young patients does not have a more aggressive course than that in older patients. Comparison of 3-Year Outcomes for Coronary Artery Bypass Graft Surgery and Drug-Eluting Stents: Does Sex Matter?
CONCLUSIONS: For women, the advantage of CABG surgery over DES is very similar to what was found for men, and this advantage persisted for patients with and without high-risk characteristics. PMID: 26294345 [PubMed - as supplied by publisher]
