Off-pump CABG surgery in left main coronary artery disease: a single-center prospective registry
ConclusionsThis study shows that off-pump CABG surgery is safe in patients with LM CAD. There is low in-hospital mortality and MACE and 3-year outcomes are similar to the published data of LM CAD patients who undergo on-pump CABG.

Concomitant occlusion of the LAA during cardiac surgery is found to have reduced postoperative stroke. Early and midterm outcomes after off pump coronary artery bypass surgery
ConclusionOPCAB can safely be performed with satisfactory short and mid-term outcomes. Further corroborative studies from different regions of the country or a multi-center study will help to establish the suitability of the technique in Indian patients.

Incidence, predictors, and outcome for post-operative atrial fibrillation in Indian patients undergoing off-pump coronary artery bypass grafting —a prospective observational study
ConclusionsIn our study, the incidence of POAF was much lower (7.94%) than that reported previously. POAF significantly increased adverse outcomes and length of hospital stay. Both EuroSCORE II and STS scores had similar discriminating power in predicting POAF.

Fractional flow reserve –guided PCI as compared with coronary bypass surgery (FAME III)– another brick in the wall!
AbstractFAME III, a multi-centric randomized controlled trial compared fractional flow reserve (FFR) guided percutaneous coronary interventions (PCI) with coronary artery bypass grafting. The study confirmed that compared with FFR-guided-PCI, coronary artery bypass grafting was associated with a significantly lower incidence of the composite of mortality, stroke, MI, and repeat revascularizations at 1 year. Thus, FFR-guided-PCI failed to meet the “noninferiority” criteria.

Short- and Long-term cardiovascular outcomes in insulin-treated versus non-insulin-treated diabetes mellitus patients after percutaneous coronary intervention: a systematic review and meta-analysis

AbstractThe Nordic –Baltic–British left main revascularization trial (NOBLE) is a prospective, randomized, multicentre, non-inferiority trial comparing percutaneous coronary angioplasty (PCI) with coronary artery bypass grafting (CABG) for revascularization of patients with unprotected left main coronary artery (L MCA) stenosis. The primary outcome was a combined endpoint of all-cause mortality, stroke, non-procedural myocardial infarction and repeat revascularization. Patients who develop post-operative atrial fibrillation have reduced survival after off-pump coronary artery bypass grafting
ConclusionThe incidence of POAF was lower than reported in the wider global literature. Increasing age and development of post-operative sepsis were strong independent predictors of POAF. POAF increases the morbidity; length of hospital stay and these patients show decreased survival after off-pump CABG.

ConclusionEarly outcome of off-pump CABG was excellent in this study. Increased incidence of deep sternal wound infection remains a concern. Multicenter study with a larger sample size is required for a dependable evaluation of the efficacy of off-pump CABG in Indian population.

Perioperative strokes following combined coronary artery bypass grafting and carotid endarterectomy: A nationwide perspective
Conclusion: A strong association was found to exist between perioperative stroke and in-hospital mortality and morbidity after combined CABG and CEA. CEA procedures are thought to mitigate the high stroke rate of 3-5% post-CABG, but our study found that combined procedures exhibit a similar stroke risk undercutting their effectiveness. Further investigative studies on combined CABG+CEA are needed to assess risk-stratification for better patient selection and examine other preventative strategies to minimize the risk of ischemic strokes.
