The efficacy and safety of cilostazol in ischemic stroke patients with peripheral arterial disease (SPAD): protocol of a randomized, double‐blind, placebo‐controlled multicenter trial
ConclusionThe SPAD trial is the first study to evaluate the safety and efficacy of dual antiplatelet agents, aspirin plus cilostazol, in comparison with aspirin alone in patients with both ischemic stroke or TIA and PAD. Results from this trial will provide important information on the merit of adding cilostazol to aspirin for slowing down progression of atherosclerosis in patients with ischemic stroke and PAD.

FDA strengthens warning that NSAIDs increase heart attack and stroke risk
Back in 2005, the FDA warned that taking nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen and naproxen increased the risk of having a heart attack or stroke. Last week it took the unusual step of further strengthening this warning. This was done on the advice of an expert panel that reviewed new information about NSAIDs and their risks. Because NSAIDs are widely used, it’s important to be aware of downsides of taking an NSAID and to take steps to limit the risk. Many people take NSAIDs to relieve mild to moderate pain. These medications may be particularly effective in conditions in which pain results pri...

Long-Term Antiplatelet Mono- and Dual Therapies After Ischemic Stroke or Transient Ischemic Attack: Network Meta-Analysis Stroke
Conclusions Long-term monotherapy was a better choice than long-term dual therapy, and cilostazol had the best risk–benefit profile for long-term secondary prevention after stroke or transient ischemic attack. More randomized controlled trials in non–East Asian patients are needed to determine whether long-term use of cilostazol is the best option for the prevention of recurrent stroke.

Aspirin for Stroke Prevention in Elderly Patients With Vascular Risk Factors: Japanese Primary Prevention Project Clinical Sciences
Conclusions— Aspirin did not show any net benefit for the primary prevention of stroke in elderly Japanese patients with risk factors for stroke, whereas age >70 years, smoking, and diabetes mellitus were risk factors for stroke regardless of aspirin treatment. Clinical Trial Registration— URL: http://www.clinicaltrials.gov. Unique identifier: NCT00225849.

Antiplatelet Strategies and Outcomes in Patients with Noncardioembolic Ischemic Stroke from a Real-World Study with a Five-Year Follow-Up
In conclusion, clopidogrel is suggested for patients with an ESRS >  3 and aspirin for patients with an ESRS ≤ 3 for the secondary prevention of noncardioembolic ischemic stroke in Chinese patients.

CYP2C19 Loss-of-Function Associated with First-Time Ischemic Stroke in Non-surgical Asymptomatic Carotid Artery Stenosis During Clopidogrel Therapy
This study measures effect ofCYP2C19 genotype on ischemic stroke risk during clopidogrel therapy for asymptomatic, extracranial carotid stenosis patients. Using deidentified electronic health records, patients were selected for retrospective cohort using administrative code for carotid stenosis, availability ofCYP2C19 genotype result, clopidogrel exposure, and established patient care. Patients with intracranial atherosclerosis, aneurysm, arteriovenous malformation, prior ischemic stroke, or observation time<1 month were excluded. Dual antiplatelet therapy patients were included. Patients with carotid endarterectomy or ...

Use of antiplatelet drugs in stroke prevention: time for a rethink?
Worldwide, 15 million people suffer stroke each year from which a third die and a further third sustain severe disability, with ischaemic events accounting for ~80% of all strokes.1 The annual estimated economic burden of stroke is 64.1 billion in Europe alone.2 While recent major investments in acute stroke services have focused on thrombolysis and hospitalisation in dedicated stroke wards, we believe that primary and secondary stroke prevention should remain of utmost importance in a disease that carries such serious permanent neurological sequelae. Antiplatelet agents such as aspirin, diypridamole or clodpidogrel are ph...

Effect of addition of clopidogrel to aspirin on stroke incidence: Meta‐analysis of randomized trials
ConclusionsThis meta‐analysis demonstrates a substantial relative risk reduction in stroke by clopidogrel plus aspirin vs. aspirin alone that is consistent across different trial cohorts. Major haemorrhage is increased by dual antiplatelet therapy.

Platelet‐Oriented Inhibition in New TIA and Minor Ischemic Stroke (POINT) Trial: Rationale and design
DiscussionAspirin is the most common antithrombotic given to patients with a stroke or transient ischemic attack, as it reduces the risk of subsequent stroke. This trial expects to determine whether more aggressive antithrombotic therapy with clopidogrel plus aspirin, initiated acutely, is more effective than aspirin alone.

Ischemic Stroke and Intracranial Hemorrhage With Aspirin, Dabigatran, and Warfarin: Impact of Quality of Anticoagulation Control Clinical Sciences
Conclusions— In Chinese patients with atrial fibrillation, the benefits of warfarin therapy for stroke prevention and ICH risk are closely dependent on the quality of anticoagulation, as reflected by TTR. Even at the top TTR quartile, warfarin was associated with a higher stroke and ICH risk than dabigatran.

Ischemic Stroke as the Presenting Symptom in a Young Patient with Essential Thrombocythemia Caused by CALR Gene Mutation (P2.253)
CONCLUSIONS: Ischemic stroke is a rare complication of essential thrombocythemia and this case is unusual because of multiple strokes in a very young patient as the presenting symptom. Fortunately, the condition was recognized and successfully treated with aspirin and hydroxyurea. She was not left with any residual neurological deficits. This case demonstrates another unusual and rarely reported cause for stroke in a young patient.Disclosure: Dr. Anderson has nothing to disclose. Dr. Bonsell has nothing to disclose.

Antiplatelet Regimen for Patients With Breakthrough Strokes While on Aspirin Brief Report
Conclusions—Among patients who experience an ischemic stroke or transient ischemic attack while on aspirin monotherapy, the addition of or a switch to another antiplatelet agent, especially in the first days after index event, is associated with fewer future vascular events, including stroke.
