Distinction between diagnostic and classification criteria: a comment on ‘Biopsy negative giant cell arteritis – Revised diagnostic criteria: Giant cell arteritis diagnostic criteria’
We read with interest the case reported by Dr. Paquale Finelli1 which highlighted the importance of evaluating patients with non-invasive arterial imaging to identify radiographic features of large-vessel vasculitis (LVV). We agree with the author that evaluating for extracranial LVV is advised among patients presenting with stroke, elevated inflammatory markers, systemic inflammatory symptoms, and absence of traditional vascular risk factors; particularly if a thromboembolic source is not identified.

Giant cell arteritis with stroke and systemic diffuse vasculitis
C Lu, S Zhou, C Yang, J YeJournal of Postgraduate Medicine 2022 68(4):233-235 Giant cell arteritis (GCA) is a systemic vasculitis, which mainly affects the aorta and its branches, while the abdominal aorta, the iliac artery and the limbs involvement are rare. The common clinical manifestations include headache, fever, jaw claudication, and visual impairment. Cerebrovascular accidents are relatively rare. Here, we present a GCA patient with stroke and systemic diffuse vasculitis involving the abdominal aorta, iliac artery, and femoral artery simultaneously.

Premature stroke and cardiovascular risk in primary Sj ögren's syndrome
CONCLUSION: Raynaud's phenomen as well as vasculitis and high ESSDAI have shown a significant association to CVD. PSS patients with cerebrovascular events were younger than expected. Knowledge about risk factors may help clinicians to identify pSS patients at risk for CVD. After diagnosis of pSS, patients should be screened for risk factors such as hypertension and receive appropriate therapy to prevent or at least reduce sequelae such as infarction. However, further investigations are necessary in order to achieve a reliable risk stratification for these patients.PMID:36588566 | PMC:PMC9794609 | DOI:10.3389/fcvm.2022.1048684
