Embolic infarct topology differs between atrial fibrillation subtypes and embolic stroke of undetermined source
CONCLUSION: Cortical infarctions were twice as common among patients with AF versus ESUS. Subcortical infarct topography was strongly associated with chronic microvascular ischemic changes and therefore may not represent embolic phenomena. Larger-scale investigations are warranted to discern whether large or multifocal subcortical infarcts ought to be excluded from the ESUS designation.PMID:36130470 | DOI:10.1016/j.jstrokecerebrovasdis.2022.106782

Serum circulating sirtuin 6 as a novel predictor of mortality after acute ischemic stroke
In this study, we aimed at evaluating the prognostic value of serum SIRT6 levels in patients with acute ischemic stroke (AIS). Serum levels of SIRT6, collected within 72 h from symptom-onset, were measured in 317 consecutively enrolled AIS patients from the COSMOS cohort. The primary endpoint of this analysis was 90-day mortality. The independent prognostic value of SIRT6 was assessed with multivariate logistic and Cox proportional regression models. 35 patients (11%) deceased within 90-day follow-up. After adjustment for established risk factors (age, NIHSS, heart failure, atrial fibrillation, and C reactive protein), SIR...

Contaminant Metals as Cardiovascular Risk Factors: A Scientific Statement From the American Heart Association
J Am Heart Assoc. 2023 Jun 12:e029852. doi: 10.1161/JAHA.123.029852. Online ahead of print.ABSTRACTExposure to environmental pollutants is linked to increased risk of cardiovascular disease. Beyond the extensive evidence for particulate air pollution, accumulating evidence supports that exposure to nonessential metals such as lead, cadmium, and arsenic is a significant contributor to cardiovascular disease worldwide. Humans are exposed to metals through air, water, soil, and food and extensive industrial and public use. Contaminant metals interfere with critical intracellular reactions and functions leading to oxidative st...

Association of Mild Valvular Lesions With Long-term Cardiovascular Outcomes Among Black Adults
CONCLUSIONS AND RELEVANCE: Results of this study indicate an association between valvular lesions, even at mild stage, and a long-term risk of cardiovascular events, suggesting the importance of recognizing and monitoring these valvular conditions.PMID:35552723 | DOI:10.1001/jamanetworkopen.2022.11946

Questionnaire-based exposome-wide association studies (ExWAS) reveal expected and novel risk factors associated with cardiovascular outcomes in the Personalized Environment and Genes Study
In conclusion, using statistics and machine learning, these findings identify novel potential risk factors for CVD, enable hypothesis generation, provide insights into the complex relationships between risk factors and CVD, and highlight the importance of considering multiple exposures when examining CVD outcomes.PMID:35605674 | DOI:10.1016/j.envres.2022.113463

The relationship between lipoprotein(a) and risk of cardiovascular disease: a Mendelian randomization analysis
CONCLUSION: This MR study suggested a causal relationship between Lp(a) and CHD, AA, and ISL, but not associated with AF, HF, IS, LIS, ISS, HBP, or PE. Our work further verifies the association between Lp(a) and various CVD, resulting in improved Lp(a) management and a reduction in the prevalence of CVD.PMID:36303257 | DOI:10.1186/s40001-022-00825-6

Plasma TSH and cardiovascular disease in the general population: A Mendelian randomization study of 105,224 individuals
CONCLUSIONS: In 105,224 individuals from the general population low plasma TSH was observationally and genetically associated with increased risk of AF, MI, and AVS suggesting that these observations may reflect causal pathways.PMID:37263031 | DOI:10.1016/j.atherosclerosis.2023.05.018

Left ventricular assist device implantation combined with hemiarch replacement for severe aortic atherosclerosis
J Thorac Dis. 2023 Jul 31;15(7):3979-3983. doi: 10.21037/jtd-23-255. Epub 2023 Jul 6.ABSTRACTImplantation of the left ventricular assist device (LVAD) has emerged as a widely employed therapeutic approach for specifically chosen individuals suffering from severe heart failure. Stroke is a well-known complication of LVAD implantation. Concomitant aortic surgeries in patients requiring LVAD implantation to avoid outflow graft anastomosis to a severe atherosclerotic aorta are unknown. We illustrate a successful LVAD implantation with hemiarch replacement for severe aortic atherosclerosis to decrease the risk of postoperative ...

Inter-arm systolic blood pressure difference: non-persistence and association with incident cardiovascular disease in the Multi-ethnic Study of Atherosclerosis
J Hum Hypertens. 2022 Mar 16. doi: 10.1038/s41371-022-00669-x. Online ahead of print.ABSTRACTWe investigated inter-arm systolic blood pressure (sIAD) difference, reproducibility, and incident cardiovascular disease (CVD). We hypothesized that higher sIAD values have low prevalence and nonpersistence over years, but that CVD risk is higher starting from the time of first high absolute sIAD. In Multi-Ethnic Study of Atherosclerosis participants (n = 6725, 53% female, 45-84 years old), Doppler systolic blood pressure (SBP) measurements were made in both arms (10-minute interval) thrice over 9.5 years. Proportional hazards for...
