Time in therapeutic range of anticoagulation among patients with atrial fibrillation and cerebral amyloid angiopathy
Proc (Bayl Univ Med Cent). 2021 Dec 28;35(2):162-167. doi: 10.1080/08998280.2021.2013393. eCollection 2022.ABSTRACTAtrial fibrillation (AF) and cerebral amyloid angiopathy (CAA) present risks of ischemic stroke and intracerebral hemorrhage (ICH). Vitamin K antagonist use is associated with fluctuations in international normalized ratio (INR), which predispose to a higher bleeding risk. Patients with a diagnosis of AF and ICH while on a vitamin K antagonist were identified using the Rochester Epidemiology Project. Sixty patients were identified (mean age 81.3 years; 24 men). Thirty-three (55%) exhibited characteristics cons...

Concomitant Use of NSAIDs or SSRIs with NOACs Requires Monitoring for Bleeding.
CONCLUSION: When NSAIDs or SSRIs are required for NOAC users with AF, physicians need to monitor bleeding events and consider the use of PPIs, especially for combined use of both drugs or when initiating NOACs treatment. PMID: 32882758 [PubMed - in process]

Insomnia in elderly people.
Abstract An elderly man presented with chronic sleep problems that had recently worsened. He had evidence of disseminated vascular disease with residual left-sided weakness following a stroke. The examination was otherwise unremarkable, but his metabolic profile revealed electrolyte and vitamin deficiencies and an elevated parathyroid hormone level. Electrolytes and vitamins are critical for maintaining physiological balance. Facilitating sleep is especially important in older people for gait stability and sustaining emotional and physical health. Addressing these issues in this patient resulted in improvement, ju...

Role of Biological Markers for Cerebral Bleeding Risk STRATification in Patients with Atrial Fibrillation on Oral Anticoagulants for Primary or Secondary Prevention of Ischemic Stroke (Strat-AF Study): Study Design and Methodology.
Conclusions: The Strat-AF study may be an essential step towards the exploration of the role of a combined clinical biomarker or multiple biomarker models in predicting stroke risk in AF, and might sustain the incorporation of such new markers in the existing stroke prediction schemes by the demonstration of a greater incremental value in predicting stroke risk and improvement in clinical outcomes in a cost-effective fashion. Atrial Fibrillation and Stroke. A Review on the Use of Vitamin K Antagonists and Novel Oral Anticoagulants.
Abstract Atrial fibrillation (AF) is the most common arrhythmia, ranging from 0.1% in patients <55 years to >9% in octogenarian patients. One important issue is represented by the 5-fold increased ischemic stroke risk in AF patients. Hence, the role of anticoagulation is central. Until a few years ago, vitamin K antagonists (VKAs) and low molecular weight heparin represented the only option to prevent thromboembolisms, though with risks. Novel oral anticoagulants (NOACs) have radically changed the management of AF patients, improving both life expectancy and life quality. This review aims to summarize the mo...

Label Adherence for Non-Vitamin K Antagonist Oral Anticoagulants in a Prospective Cohort of Asian Patients with Atrial Fibrillation.
CONCLUSION: In real-world practice, more than one-third of patients with NOAC prescriptions received an off-label reduced dose, which could result in an increased risk of stroke. Considering the high risk of stroke in these patients, on-label use of NOAC is recommended. PMID: 30799590 [PubMed - in process]

Risk Factors, Co-Morbidities and Treatment of In-Hospital Patients with Atrial Fibrillation in Bulgaria.
CONCLUSIONS: Atrial fibrillation was highly prevalent among our study population. Reduced and mid-range left ventricular ejection fraction, valvular heart disease, and HTN were the risk factors with the strongest association with AF. Although a large number of our AF patients were administered antithrombotic treatment, the prescription rate of oral anticoagulants should be further improved. PMID: 30344265 [PubMed - in process]

Development and Comparison of Warfarin Dosing Algorithms in Stroke Patients.
CONCLUSION: Our warfarin dosing algorithm may be useful for Korean patients with stroke. Further studies to elucidate clinical utility of genotype-guided dosing and find the additional genetic association are necessary. PMID: 26996562 [PubMed - in process]
