Epidemiology, repair technique, and predictors of stroke and mortality in penetrating carotid artery injuries
This retrospective review of the NTDB found that carotid repair was associated with improved mortality outcomes compared to ligation; however, there was no difference in stroke outcome. The factors predictive of stroke were low GCS, high ISS, and a history of neurological deficit prior to injury and those predictive of in-hospital mortality were carotid ligation, low GCS, high ISS, and cardiac arrest.

Early Thoracic Endografting is Safe in Patients with Uncomplicated Type B Aortic Dissection
Zentralbl Chir. 2023 May 30. doi: 10.1055/a-2060-9864. Online ahead of print.ABSTRACTThere are many publications dealing with treatment options for uncomplicated type B aortic dissection (TBAD). Early TEVAR (Thoracic Endovascular Aortic Repair) may improve aortic-specific survival and delay disease progression in the long-term. Especially in patients with uncomplicated TBAD and additional high-risk features, preemptive TEVAR may improve late outcomes. We conducted a retrospective analysis of all patients treated for TBAD in our hospital between February 2017 and September 2021. Comorbidities, intraoperative data, 30-day mor...

Effect of the Pre-Operative Systemic Inflammatory Response on Outcomes Following Carotid Endarterectomy for Symptomatic Carotid Stenosis
Objective: Stroke accounts for over 100 000 acute hospital presentations per year in the UK, and approximately 4 500 carotid endarterectomies are performed annually in the UK.1 This is despite a relative paucity of cases performed for asymptomatic carotid stenosis, in comparison with other countries.1 Periprocedural stroke and death rates have fallen in recent years,2 although these remain notable risks necessitating careful patient selection and counselling. Chronic activation of the systemic inflammatory response (SIR) is prevalent in patients with atherosclerotic disease and influences both the pathogenesis and prognosis.

Distal embolic protection use during transfemoral carotid artery stenting is associated with improved in-hospital outcomes
Despite current guidelines recommending the use of distal embolic protection during transfemoral carotid stenting (tfCAS) to prevent periprocedural stroke, there remains significant variation in the routine use of distal filters. We sought to assess in-hospital outcomes in patients undergoing tfCAS with and without embolic protection using a distal filter.

Effect of Postoperative Stroke Timing on Perioperative Mortality after Carotid Revascularization
In-hospital stroke (IHS) has been associated with worse outcomes than out-of-hospital stroke (OHS) due to delays in diagnosis and treatment. A paucity of studies exist comparing the timing of postoperative stroke after carotid revascularization. We aimed to study the effect of IHS vs. OHS on postoperative mortality in carotid revascularization patients in a large-scale national database.

Qualitative Analysis of Length of Stay and Readmission After Carotid Endarterectomy
The objective of this study was to evaluate factors related to hospital length of stay (LOS) and readmission within 90 days following carotid endarterectomy (CEA) in patients who have not had a stroke.

Which Patients Receive Diagnostic Angiography? An EAST Multicenter Study Analysis of Internal Carotid Artery Blunt Cerebrovascular Injury
DISCUSSION: DSA is frequently used in ICA BCVI, primarily early in the hospital course for injury diagnosis and grade determination. DSA appears primarily driven by hospital type, BCVI grade, and luminal stenosis.PMID:36417771 | DOI:10.1177/00031348221138083

In-hospital outcomes after carotid endarterectomy for stroke stratified by modified Rankin scale score and time of intervention
Although the benefits of carotid endarterectomy (CEA) for treating symptomatic carotid stenosis are well known, the optimal timing of intervention after acute stroke and whether the optimal timing will vary with preoperative stroke severity has remained unclear. Therefore, we assessed the effect of stroke severity and timing of the intervention on the postoperative outcomes for patients who had undergone CEA for stroke.
