Comments on Stroke as a Neurological Complication of COVID-19: A Systematic Review and Meta-Analysis of Incidence, Outcomes and Predictors
We read with interest the research article entitled “Stroke as a Neurological Complication of COVID-19: A Systematic Review and Meta-Analysis of Incidence, Outcomes and Predictors”,1 it was a systematic review and meta-analysis on stroke incidence and different outcomes over COVID-19 patients. We are particularly concerned about the potential gro ups of overlapped COVID-19 stroke subjects in the meta-analysis under the worldwide SARS-CoV-2 pandemic.

COVID-19 encephalitis with SARS-CoV-2 detected in cerebrospinal fluid presenting as a stroke mimic
We report the case of a 35-year-old male with COVID-19 encephalitis presenting as a stroke mimic with sudden-onset expressive and receptive dysphasia, mild confusion and right arm incoordination. The patient received thrombolysis for a suspected ischaemic stroke, but later became febrile and SARS-CoV-2 was detected in cerebrospinal fluid. Electroencephalography demonstrated excess in slow waves, but neuroimaging was reported as normal. Respiratory symptoms were absent throughout and nasopharyngeal swab was negative for SARS-CoV-2.

Impact of COVID-19 pandemic on stroke severity and mortality in the south-east of Santiago, Chile
Background and purpose: Chile has been one of the most affected countries by the COVID-19 pandemic, with one of the highest case rates per population. This has affected the epidemiological behaviour of various pathologies. We analyze the impact of the pandemic on the number of admissions due to stroke, its severity and mortality in Santiago, Chile.Methods: a multicenter observational study based on the records of the 3 hospitals of the South East health service in Santiago, Chile. We recorded the number of patients admitted for ischemic stroke between 01 January 2020 and 30 June 2020.

Utilization and Outcomes of Acute Revascularization Treatment in Ischemic Stroke Patients with SARS-CoV-2 Infection
Our understanding of outcomes in acute ischemic stroke patients with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection following acute revascularization treatments (as intravenous thrombolysis and/or thrombectomy) is based on small case series.1-3. An international panel4 cautioned regarding the potential for high rate of death or disability and post thrombolytic intracerebral hemorrhage (ICH)s in acute ischemic stroke patients with SARS-CoV-2 infection due to elevated concentrations of inflammation and hypercoagulability markers such as leukocytosis, and C reactive protein and D dimers and multisystem dysfunction.

Cryptogenic, Embolic Stroke —Looking Backstage
Stroke is a common and devastating event and the majority of cases are caused by thromboembolism from the left atrium, left ventricle or left sided valves. This case report describes a case of embolic stroke with the origin of the thrombus from the left inferior pulmonary vein. The importance of this case is twofold. Firstly, it is the fourth case report of pulmonary venous thrombosis, a very rare condition, due to COVID-19 infection and secondly, it focuses attention on the fact that the left atrium is not the most proximal address of arterial thromboembolism —the pulmonary veins are.

Clinicians and researchers have addressed concerns about the negative impact of COVID-19 outbreaks on the ability of health care systems to provide timely assessment and acute therapies to patients with stroke. The aim of this study is to describe stroke care during the first wave of the COVID-19 pandemic compared to the same period the year before at an acute care hospital in Sweden.
