Preprocedural determination of an occlusion pathomechanism in endovascular treatment of acute stroke: a machine learning-based decision
Conclusions An ML-PM could accurately determine an occlusion pathomechanism with common preprocedural findings. A decision flowchart consisting of the four most influential findings was clinically applicable and superior to single common preprocedural findings for determining an occlusion pathomechanism.

O06/60 Diagnosis of intracranial atherosclerosis related large vessel occlusion before endovascular treatment
ConclusionA combination of clinical and radiological variables available before EVT can accurately predict the presence of an ICAD-LVO. The ICAD-Scale could be useful to perform a rapid assessment of underlying etiology and suggest specific pathophysiology-based measures (adjunctive pharmacological treatment, angioplasty and/or intracranial stenting).Disclosure of InterestDr Molina reported receiving personal fees from AstraZeneca for consultant services outside the submitted work. O-008 Endovascular thrombectomy outcomes stratified by hospital thrombectomy volume: a national inpatient sample study
ConclusionsPatients receiving EVT for AIS at HV centers were more likely to be discharged to home without assistance but demonstrated lower rates of in-hospital mortality when compared to IV and LV centers. Our findings support HV EVT performing centers render more favorable outcomes in this NIS analysis.Disclosures A. Brake: None. L. Fry: None. C. Heskett: None. G. LeBeau: None. C. Lei: None. A. P-005 Predictors for large vessel recanalization before stroke thrombectomy
ConclusionsIVT at least 1.5 hours before angiography, site of vascular occlusion, atrial fibrillation, and hyperlipidemia are independent predictors for LVR. The 8-point Recan score proposed in this study may be a valuable tool for predicting LVR before EVT.Abstract P-005 Figure 1Disclosures H. Chen: None. M. Colasurdo: None. C. Schrier: None. M. E-245 Association of pre-treatment cerebral microbleeds with hemorrhagic transformation following successful endovascular recanalization in patients with acute ischemic stroke arising from large artery occlusion
ConclusionsThese findings suggest that the presence of CMBs is associated with HT and poor functional outcome following EVT and had no effect on mortality in patients with successful endovascular recanalization. However, CMBs do not show such a high impact on HT as other literature in our study. Therefore, it seems appropriate to consider CMB status in individual risk stratification of poor clinical outcome following recanalization by EVT with AIS. Future large and well-designed prospective studies are required to better characterize the underlying factors behind CMB-associated clinical deterioration.Abstract E-245 Figure ...

Intravenous thrombolysis before mechanical thrombectomy in patients with atrial fibrillation
The introduction into clinical practice of endovascular mechanical thrombectomy (MT) has brought better results in patients with ischemic stroke from large vessel occlusion (LVO) in the anterior circulation in whom therapy is started 4.5–5 hours after symptoms onset.1–3 Controversies exist about the usefulness of intravenous thrombolysis (IVT) before MT in patients with atrial fibrillation (AF). AF is the probable etiology in >25% of ischemic strokes, and >40% of patients with ischemic stroke from LVO in the anterior circulation have a clinical history of AF.4 5 AF is an independent predictor of poor func...

Bridging thrombolysis in atrial fibrillation stroke is associated with increased hemorrhagic complications without improved outcomes
Conclusion In this large thrombectomy registry, AF patients exposed to IVT before MT had increased hemorrhagic complications without improved functional outcomes, in contrast with non-AF patients. Prospective trials are warranted to assess whether AF patients represent a subgroup of LVO patients who may benefit from a direct to thrombectomy approach at thrombectomy capable centers.

Predictors of futile recanalization after endovascular treatment in acute ischemic stroke: a meta-analysis
Conclusion This study identified female sex, comorbidities, admission systolic blood pressure, serum glucose, occlusion site, non-bridging therapy, and post-procedural complication as predictors of futile recanalization, and also confirmed previously reported factors. Further large-scale prospective studies are needed.

O-016 Outcomes after endovascular mechanical thrombectomy for low national institutes of health stroke scale (NIHSS): a multicenter study
ConclusionsMT is an effective and safe treatment for appropriately selected ischemic stroke patients presenting with low NIHSS. Diabetes and prior stroke are predictors of functional dependence at 90 days, and prior stroke is also associated with mortality, suggesting that MT should be offered judiciously to patients with these risk factors. Our findings signal the need for a randomized trial comparing MT versus medical management for LVO patients with low NIHSS.Disclosures I. Abecassis: 2; C; IschemaView (Rapid), Remedy Robotics. E.
