Characteristics of Recurrent Ischemic Stroke After ESUS
This secondary analysis of a randomized clinical trial assesses the subtype of recurrent ischemic stroke after embolic stroke of underdetermined source, the interaction with treatment assignment, and the consistency of cerebral location of the qualifying and recurrent strokes in the study population of the NAVIGATE-ESUS trial.

Questions on Predicting Early Neurological Deterioration in Patients With Minor Stroke and Large-Vessel Occlusion
To the Editor We read with great interest the article by Seners and colleagues in which they investigated predictors of early neurological deterioration of presumed ischemic origin (ENDi) in large-vessel occlusion stroke with minor clinical deficits and developed a 4-point risk prediction score of ENDi. This is a common and probably underrecognized problem in large-vessel occlusion stroke, and characterizing patients at risk of ENDi is highly relevant for patient management.

Infarct on Brain Imaging, Subsequent Ischemic Stroke, and Clopidogrel-Aspirin Efficacy
This post hoc analysis of a randomized clinical trial investigates whether the association of clopidogrel-aspirin treatment with stroke recurrence in patients with minor stroke or high-risk transient ischemic attack is modified by the presence of infarct on imaging attributed to the index event.

Data Do Not Support Selection of Patients for Stroke Treatment Within the 16- to 24-Hour Interval —Reply
In Reply We thank Powers for his comments regarding our article. He raises the question if the positive result for patients with target perfusion-imaging mismatch in the 12.8- to 24-hour tercile in the Analysis of Pooled Data From Randomized Studies of Thrombectomy More Than 6 Hours After Last Well Known (AURORA) study could solely be due to a benefit from 12.8 to 16 hours since last known well subgroup, with neutral or negative effects in the 16- to 24-hour time period. This was not the case. Data for patients in the 16- to 24-hour group with the target mismatch profile demonstrated an odds ratio of 3.82 (95% CI, 1.07-13....
