Abstract 210: Association of Emergency Medical Services Stroke Protocols with Minimizing On-Scene Time for Stroke Patients Poster Session II
Conclusion: Specific scene time limits were associated with EMS crews spending less time at the scene while general instructions were not. These findings suggest that EMS stroke protocols that include specific scene time limits are more likely to reduce on-scene times than those with no or only general instructions.

Abstract 240: Regional Variation in 30-Day Ischemic Stroke Mortality and Readmissions in Get With the Guidelines-Stroke Hospitals Session Title: Abstract Poster Session II
Conclusions: HRR-level 30-day ischemic stroke mortality and readmission rates exhibit substantial regional variation and are inversely correlated. Further research is ongoing to determine the origin of the regional variation and correlation between 30-day event rates.

Abstract 4: Variation in Published Stroke Rates Results in Wide Variation in the Net Clinical Benefit of Anticoagulation for Atrial Fibrillation Session Title: QCOR 2018 Young Investigator Award Finalists
Conclusion: Using current guidelines, variation in published off-anticoagulation stroke rates results in tenfold variation in the estimated net clinical benefit of anticoagulation. Guidelines should better reflect the uncertainty of the current approach that uses a CHA2DS2-VASc threshold to recommend anticoagulation.

Conclusion: White older ischemic stroke patients have the highest 30-day mortality rates of any race/ethnicity group. By one year, only Asian patients have differentially lower mortality risk. In contrast, black and Hispanic patients have higher readmission risk to one year.1

Abstract 100: Effects of time to Physical Therapy Order on Length of Stay and Discharge NIH Stroke Scale Scores in an Acute Stroke Unit Poster Session I
Conclusions: Ideally, stroke patients should be evaluated for physical therapy services within 24 hours of a PO. Shortage of PT staff in a busy health care facility may impact how soon patients can be seen. Although the current pilot study did not reveal significant changes in LOS, and NIHSS scores of stroke patients at discharge irrespective of whether patients were seen by physical therapists ≤ or >48 hours after a PO, severity of the stroke, previous history of stroke or TIAs, and other co-morbidities that could have confounded the results were not examined. Abstract 235: Acute Care Measures of Rehabilitation Needs Associated with Discharge to Rehabilitation for Patients with Minor Stroke Poster Session II
Conclusions: Rehabilitation needs were documented for patients with minor stroke and almost one third were discharged to receive inpatient rehabilitation in an IRF or SNF. Taking pre-stroke disability into consideration for transition planning from acute care could facilitate earlier communication and planning with the next point of care.

Abstract 238: Utilization and Safety of a Heparin Nomogram in Treating Thrombotic Comorbidities in Stroke Patients Poster Session II
CONCLUSION: The heparin stroke nomogram appears safe in that it does not increase risk of ICH. However, therapeutic aPTTs are not reached quickly with less than half of patients therapeutic at 24 hours, nor are they maintained at steady state. This nomogram could be too conservative in cases where clinical situation requires rapid anticoagulation.

Validity of Claims-Based Stroke Algorithms in Contemporary Medicare Data: Reasons for Geographic and Racial Differences in Stroke (REGARDS) Study Linked With Medicare Claims Methods Paper
Conclusions— Claims-based algorithms to identify stroke in a contemporary Medicare cohort had high positive predictive value and specificity, supporting their use as outcomes for etiologic and comparative effectiveness studies in similar populations. These inpatient algorithms are unsuitable for estimating stroke incidence because of low sensitivity.

Conclusions: The use of quality improvement approaches such as six-sigma can result in significant improvement in DTN. The data-driven approach of six-sigma allowed the Calgary Stroke Program to identify and improve various processes that effect the DTN time, which resulted in a lower average DTN time and a reduction in variation.

Abstract 270: Primary and Specialty Care Follow-up Appointments after Stroke: Rates, Variability, and Opportunities for Improvement Session Title: Poster Session II
Conclusions: A high proportion of patients with acute stroke failed to have a primary care or specialty care follow-up appointment scheduled prior to discharge and this varied widely across participating hospitals. A collaborative approach for data-driven change and exchange of local best practices and facilitators could lead to significant improvement.

Abstract 203: Community Based Outpatient Stroke Rehabilitation Program Achieves Excellent Outcomes Including Return to Work, Driving, Improved Blood Pressure Control, and Other Rehabilitation Outcomes Session Title: Poster Session II
Conclusions: Participants in our interdisciplinary community-based rehabilitation program demonstrated improvements in stroke knowledge, physical function, blood pressure control, and mood, while also returning to work, driving, and participation in the community. Comparative research and multi-site studies are needed.

Abstract 6: Treatment Effect of Clopidogrel Plus Aspirin Within 12 Hours of Acute Minor Stroke or TIA Session Title: Abstract Oral Session: Outcomes
Conclusions: Among patients treated within 12 hours, the combination of clopidogrel and aspirin was more effective than aspirin alone in reducing the risk of recurrent ischemic stroke during the 90-day follow-up and did not increase the hemorrhagic risk.
