Priorities for Comparative Effectiveness Reviews in Cardiovascular Disease Original Articles
Conclusions— Using a systematic process deriving consensus from multiple stakeholders across cardiovascular disease states, we generated a prioritized list of evidence synthesis topics to inform decision makers. The topics vetted through this process seek to determine the comparative safety and effectiveness of a range of treatments, both established and emerging, and are immediately relevant for prevalent disease states.

Abstract 2: The Changing Face of Cardiac Surgery: Frailty, Age, and Adverse Outcomes Create a Mandate for Shared Decision Making Concurrent Session I Session A: Oral Abstract Presentations on Less is More
CONCLUSIONS We have identified increasing rates of elderly and frail patients with high-risk profiles referred for cardiac surgery. These patients experience higher rates of mortality and prolonged institutional care. We suggest this vulnerable patient population may benefit from the institution of a formalized shared decision making process to effectively communicate risks, benefits and alternatives to the planned procedure.

Conclusions: There is a fundamental difference in values surrounding death among patients who either accept or decline a DT LVAD. Decision support interventions around this complex medical decision need to consider patient values first before getting into more cognitive aspects of the decision making process.

CV events among T2DM patients are major contributors to high healthcare utilization and cost. Healthcare utilization and costs among T2DM patients with established CVD and CVD risk factors in a large US database were explored.T2DM patients aged ≥40 years were identified from the HealthCore Integrated Research Environment from 1/1/2007 to 4/30/2011 and followed from first T2DM diagnosis (index date) until loss of eligibility or death. Conclusion: Our simple decision rule could significantly reduce the number of stress tests ordered by primary care physicians without compromising event-free survival. Further refinement and validation of this score in both retrospective and prospective studies is warranted.

Abstract 63: The Smoking Paradox in Patients Hospitalized with Coronary Artery Disease: Findings from Get With The Guidelines - CAD Poster Session I
Conclusion: Smoking continues to be a major risk factor for presenting with CAD at a much younger age and with fewer risk factors. It is likely that the continued modest association with lower in-hospital mortality in smokers in this analysis after adjustment reflects residual or unmeasured confounding. This apparent smoker’s paradox in CAD should not be interpreted as a benefit of cigarette smoking.

Abstract 65: Identifying and Adapting a Prognostic Risk Equation for Relevant Subpopulations in a Pharmacoeconomic Model of Major Adverse Cardiovascular Event Prevention Poster Session I
Conclusions: Estimating the ratio of absolute risks for specific populations and subsets using QRISK2 can help a model to predict the value of targeting relevant subpopulations for a new treatment. When this risk adjustment is included in a lifetime model, additional benefits from secondary prevention, and increased likelihood that a treatment is cost effective in a UK context, can also be estimated. For non-UK populations, calibration of QRISK2 is needed or an alternative equation must be used.

Conclusions: While the vast majority of AF encounters initiated in the ED result in admission, encounters where patients were treated with DE as continuing or new therapy were less likely to be admitted compared to similar encounters where warfarin was administered.

Abstract 96: Sex Differences in Pre-Event Health Status of Young Patients with Acute Myocardial Infarction: The VIRGO Study Poster Session I
Conclusion: Compared with men, young women presenting with an AMI seem to have worse health status with poorer physical functioning and quality of life prior to their event. In light of these findings, it may be useful for healthcare providers to identify young women who are at increased cardiovascular risk at an earlier stage in order to optimize their health status.

Conclusions: There is a growing need to develop care strategies for adults with congenital heart disease. By developing a focus of care at pediatric hospital, adults received outstanding surgical results with very little morbidity or mortality.

Abstract 150: Resource Utilization Patterns and Outcomes Among Acute Coronary Syndrome Patients: Findings From a Multi-Payer Analysis Poster Session II
Conclusion: These findings suggest that many patients with ACS do not receive appropriate recommended antithrombotic prophylaxis, and opportunities exist to improve therapy. Increased use of software tools such as AQuA may support enhanced education efforts aimed at improving adherence to guidelines and quality of care.

Abstract 185: Health Care Resource Utilization of Acute Coronary Syndrome (ACS) in a Geographically Distributed US Population 2006-2011 Poster Session II
Conclusion: Patients with recurrent CVEs had higher 1- and 3-year post-index overall and CV-related utilization as compared with patients without recurrent CVEs. More aggressive strategies to manage this increased long-term utilization is warranted. This study underscores the need to prevent subsequent adverse CVEs, ultimately to improve patient outcomes and help reduce overall health care utilization.

Abstract 205: New Onset Heart Failure and Adverse Ischemic Events Associated with Amiodarone and Dronedarone Use among Atrial Fibrillation Patients Poster Session II
Conclusions: In a propensity score matched observational cohort study, amiodarone use was associated with higher incidence rate of new onset HF, HF hospitalizations, and TIA as identified from claims. This finding differs from other clinical studies. Future observational cohort studies should incorporate medical record review for validation since information from claims might be insufficient to fully account for underlying patient risk status, or accurately determine if HF was new onset.Key words: amiodarone; dronedarone; atrial fibrillation; heart failure.
