Physical activity in vascular cognitive impairment: Systematic review with meta-analysis
Vascular cognitive impairment (VCI) is the second most common cause of cognitive impairment worldwide and includes a spectrum from vascular cognitive impairment no dementia (VCIND) to vascular dementia (VaD). There is no specific pharmacological treatment approved for VCI. Physical activity has been indicated to be a promising preventive measure for cognition, with direct as indirectly benefits, while improving several modifiable vascular risk factors, so potentially effective when considering VCI.

Thrombectomy in ischemic stroke patients with alberta stroke program early computed tomography score 4-5 and 0-3: Factors associated with favorable outcome
Recently published results of the ANGEL-ASPECT and SELECT2 trials suggest that stroke patients presenting with low Alberta Stroke Program Early Computed Tomography Score (ASPECTS) benefit from mechanical thrombectomy. Purpose of this retrospective study was to identify factors that are associated with a favorable outcome in patients with low ASPECTS of 4-5 and 0-3 undergoing mechanical thrombectomy.

Multiple prior studies have shown a relationship between COVID-19 and strokes; further, COVID-19 has been shown to influence both time-to-thrombectomy and overall thrombectomy rates. “Missing-piece” sign with dural arteriovenous fistula at craniocervical junction: A case report
Spinal dural arteriovenous fistula (sDAVF) is a rare and often underdiagnosed spinal disease. Early diagnosis is required because the deficits are reversible and delays in treatment cause permanent morbidity. Although the abnormal vascular flow void is a critical radiographic feature of sDAVF, they are not always present. A characteristic enhancement pattern of sDAVF has been recently reported as the “missing-piece” sign which can lead to the early and correct diagnosis.

Visual review of acute stroke neuroimaging prior to transfer acceptance increases likelihood of endovascular therapy
Demand for thrombectomy, and interhospital transfer to comprehensive stroke centers (CSCs), for acute stroke is increasing. There is an urgent need to identify patients most likely to benefit from transfer. We evaluated whether CSC providers ’ review of neuroimaging prior to transfer acceptance improved patient selection for thrombectomy and correlated with higher rates of treatment.

Intravenous thrombolysis in a patient with acute ischemic stroke associated with a ruptured sinus Valsalva aneurysm: The first case report
Sinus Valsalva aneurysms (SVA) are rare anomalies caused by weakness at the junction of the aortic media and the annulus fibrosus.1 SVAs are clinically silent until they rupture. The rupture can cause right-sided heart failure, myocardial infarction, strokes, and even sudden cardiac death.1 This is the first case report of a patient with a ruptured SVA who presented with acute ischemic stroke and was treated successfully with recombinant tissue-type plasminogen activator (tPA).

The charlotte large artery occlusion endovascular therapy outcome score compares favorably to the critical area perfusion score for prognostication before basilar thrombectomy
Late window endovascular thrombectomy (EVT) trials enrolled patients presenting with an internal carotid artery (ICA) or proximal middle cerebral artery (MCA) occlusion using computed tomography perfusion (CTP) or magnetic resonance imaging perfusion selection criteria.1,2 Radiographic1 or clinical mismatch2 was required for inclusion in these landmark studies. Although perfusion imaging is not always used for patient selection in early window anterior circulation thrombectomy,3 CTP ischemic core volume is independently associated with functional outcome improvement in the 0-6-hour time window when good endovascular reperfusion is achieved.
