Multimodal recanalization for subacute symptomatic internal carotid artery occlusion due to atherosclerosis: Outcomes and notes on hybrid techniques
Subacute internal carotid artery occlusion (ICAO) is associated with ipsilateral recurrent stroke, and successful recanalization of ICAO can improve cerebral blood perfusion (CBP) and prevent stroke. However, the optimal treatment remains controversial.

Feasibility and accuracy of measuring carotid plaque volume (burden) with contrast-enhanced tomographic 3D ultrasound and ultrasound image fusion.
Stenosis severity has been the indication for carotid endarterectomy (CEA) for four decades, but the annual stroke risk in asymptomatic carotid stenosis>70% is under 2%. Atherosclerotic volume has emerged as a risk factor for future stroke, but needs to be measured non-invasively. Tomographic ultrasound (tUS) is a novel technology that assembles 3D images in seconds. We evaluated accuracy of measuring Carotid Plaque Volume (CPV) with tUS in patients undergoing CEA.

Natural History of Carotid Artery Occlusion
Carotid artery occlusion (CAO) is a risk factor for stroke ipsilateral to the occlusion and puts patients in a high-risk category when contralateral endarterectomy is performed. The purpose of this study was to evaluate the long-term outcomes of patients with internal CAO and to determine risk factors predictive of subsequent neurological event, contralateral carotid intervention, or death. Patients with internal CAO shown by duplex ultrasonography were retrospectively identified and followed between January 2002 and June 2010 (follow-up: 1–101 months, mean: 52 months) at a tertiary care hospital. All had multiple duple...

Anesthesia-Based Evaluation of Outcomes of Lower-Extremity Vascular Bypass Procedures
Conclusion: These results provide no evidence to support the systematic avoidance of general anesthesia for lower-extremity bypass procedures. These data suggest that anesthetic choice should be governed by local expertise and practice patterns.

Primary Extracranial Vertebral Artery Aneurysms
Conclusions: Primary extracranial vertebral artery aneurysms are rare and occur in patients with hereditary disorders. Operative intervention is warranted in symptomatic patients. Exclusion and reconstruction may be performed with open and hybrid techniques with low morbidity and mortality.

Postoperative Cardiac Damage After Standardized Carotid Endarterectomy Procedures in Low- and High-Risk Patients
Conclusions: Carotid endarterectomy is followed by an increase in cTn-I value>0.5 ng/mL in 14% of all cases, although symptomatic cardiac ischemia is very low. However, high-risk patients as defined by the SAPPHIRE criteria do not show an increased risk of cardiac damage compared to low-risk patients. Larger studies using cTn-I as a marker of postoperative cardiac damage, after carotid endarterectomy or stenting, are needed.

Impact of Adding Aspirin to Beta-Blocker and Statin in High-Risk Patients Undergoing Major Vascular Surgery
In this study we evaluated the benefits of adding aspirin (A) to BB and S (ABBS), with/without angiotensin-converting enzyme inhibitor (ACE-I) on postoperative outcome in high-risk patients undergoing major vascular surgery. Methods: Analysis of consecutive patients undergoing elective vascular surgery at the University of Michigan Cardiovascular Center was performed. Univariate and multivariate analyses were done using cardiac risk index [Revised Cardiac Risk Index (RCRI), coronary artery disease (CAD), insulin-dependent diabetes mellitus (IDDM), cerebral vascular disease, renal dysfunction, congestive heart failure, and m...

New Cerebral Ischemic Lesions After Carotid Endarterectomy
Conclusions: New ischemic lesions on DW-MRI are detected in 7.5% of patients after CEA, and most of these lesions are clinically silent. Plaque echogenicity does not affect their incidence. New lesions seen on DW-MRI may be generated outside of the treated carotid artery territory.

Results of Adjunctive Spinal Drainage and/or Left Subclavian Artery Bypass in Thoracic Endovascular Aortic Repair
Conclusions: Our use of adjunctive procedures for TEVAR demonstrated better SCI results compared with those of prior reports of selective CSF drainage when SCI arises. Our approach was associated with improved 1-year survival. Preoperative CSF drain placement allows for rapid, intensive therapy for SCI and should be considered when clinically feasible.

Concomitant Unruptured Intracranial Aneurysms and Carotid Artery Stenosis: An Institutional Review of Patients Undergoing Carotid Revascularization
Conclusions: Concomitant carotid artery stenosis and UIAs is a rare entity. Carotid revascularization does not appear to increase the risk of rupture for small aneurysms (

Relation between B-mode Gray-scale Median and Clinical Features of Carotid Stenosis Vulnerability
Conclusions: No relation was found between the GSM and any clinical, radiologic, or intra- and postoperative neurologic phenomena. These data showed no additional value of the use of GSM in evaluating plaque vulnerability.

Combination of Flow Reversal and Distal Filter for Cerebral Protection during Carotid Artery Stenting
Conclusions: The additional use of a distal filter captures emboli in 17.3% of cases, and because the occlusion is only intermittent, the procedure is potentially applicable even in those who cannot tolerate prolonged balloon occlusion of the CCA.
