Transfemoral Carotid Artery Stents Should Be Used with Caution in Patients with Asymptomatic Carotid Artery Stenosis
Significant national variation exists in defining the degree of stenosis that requires intervention in patients with asymptomatic carotid artery stenosis (ACAS). We aimed to evaluate the risk of perioperative- and 2-year stroke and death in ACAS patients undergoing carotid endarterectomy (CEA) and carotid artery stenting (CAS) for severe versus very severe stenosis in a contemporary population.

Carotid Artery Entrapment by the Hyoid Bone —A Rare Cause of Recurrent Strokes in a Young Patient
We report a case of recurrent strokes in a 32-year-old woman due to an elongated hyoid bone causing thrombus formation in her right internal carotid artery (ICA), resulting in recurrent embolic strokes confirmed on diffusion-weighted magnetic resonance imaging.

Timing of Hospital-Acquired Venous Thromboembolism (VTE) and its Relationship with VTE Prevention Measures in Immobile Patients
To describe the timing of venous thromboembolism (VTE) diagnosis in patients with cerebral or spinal trauma and stroke and describe the relationships between VTE prophylaxis and timing of VTE diagnosis at a community hospital.

Carotid Endarterectomy Outcomes in the Elderly: A Canadian Institutional Experience
Carotid endarterectomy (CEA) is a well-established surgical intervention for stroke prevention in patients with carotid stenosis of all ages. However, the decision to proceed to operate in the elderly involves a more complicated risk-benefit assessment due in part, to increased comorbidities and reduced life expectancy. Some studies suggest that CEA is more risky in the elderly with worse outcomes, while others have found no difference. Our objective was to evaluate and compare outcomes of CEA between elderly and younger patients at our institution.

Impact of the Supra-Aortic Trunks And Circle of Willis Patency on the Neurological Compensation During Carotid Endarterectomy
One of the most critical phases of carotid endoarterectomy is the carotid cross-clamping test which is a concrete evaluation of efficacy of collateral cerebral perfusion. Some studies revealed a strong correlation between tolerance to carotid cross-clamping and post-operative TIA/stroke complications. The aim of the study is to make a global analysis of supra-aortic trunks and Circle of Willis patency in order to predict the tolerance to carotid cross-clamping pre-operatively.

Considerations Beyond Stenosis for Carotid Endarterectomy in Treating Free-Floating Thrombus of the Carotid Artery
Free-floating thrombus (FFT) of the carotid artery is an uncommon condition that can present with neurologic symptoms, often in the setting of ischemic stroke. The literature pertaining to the incidence and optimal treatment of this condition is limited. Herein we report our contemporary experience with FFT across a range of degrees of carotid stenosis.

Impact of Femoral Ossification on Local and Systemic Cardiovascular Patients ’ Condition
Vascular calcifications are associated with a high cardiovascular morbi-mortality when located in the coronary territory, and with atheromatous plaque stability and stroke when located in carotid arteries. Previous studies have shown that femoral arteries were more prone to calcify and to develop osteoid metaplasia (OM). So far the impact of OM on local and systemic cardiovascular events remains unknown. The aim of this study was to assess the predictive value of OM in histological analysis of femoral plaque for the occurrence of mid- and long-term adverse cardiovascular events.

Acute Occlusion of Descending Thoracic Aorta
Acute aortic occlusion is a rare but potentially devastating clinical event, that requires a prompt diagnosis and emergency treatment.Only five cases of native thoracic aorta acute occlusion have so far been reported with different pathologic causes. The clinical features depend on the level of occlusion. Sometimes, the diagnosis could be misinterpreted as a stroke or other diseases of central nervous system. This could lead to a delay in the diagnosis and revascularization procedure, followed by a morbidity or mortality increase.

Rationale and Design for the Remote Ischemic Preconditioning for Carotid Endarterectomy (RIP-CEA) Trial
While the perioperative stroke rate after carotid endarterectomy (CEA) is low, magnetic resonance imaging (MRI) “silent” microinfarctions are common and have been correlated with postoperative neurocognitive decline. Our study will investigate the role of remote ischemic preconditioning (RIPC) as a potential neuroprotective mechanism. RIPC is a well-tolerated stimulus that, through neuronal and humoral pa thways, generates a systemic environment of greater resistance to subsequent ischemic insults.
