Type III arch configuration is frequently reported as a stroke risk factor for carotid artery stenting (CAS). We reviewed contemporary guidelines on management of carotid artery stenosis to assess the clinical relevance attributed to this anatomic feature in current clinical practice.

Complexity of aortic arch anatomy affects the outcomes of transcarotid artery revascularization versus transfemoral carotid artery stenting
Transcarotid artery revascularization (TCAR) has been shown to have half the rates of transient ischemic attack (TIA), stroke and death compared to transfemoral carotid artery stenting (TFCAS). Successful outcomes of TFCAS requires careful patient selection. The aim of this study was to determine the outcomes of TFCAS versus TCAR in both simple (Type I) and complex (Type II and III) aortic arches.

The stiffness of the common carotid artery, for example, has been showed in numerous clinical studies as a marker of increased risk of stroke, dementia, and depression. Besides, with the emergence of new surgical techniques such as the transcarotid artery revascularization that uses the common carotid artery as a workstation, the biomechanical and histological features of this vessel, more than ever, must be detailed.

Surgical thromboendarterectomy for free-floating thrombus associated with cervical carotid artery dissection: A case report
Free-floating thrombus (FFT) of the cervical carotid artery is a rare but critical condition leading to stroke. The most common underlying pathology is atherosclerotic plaque; non-atherosclerotic pathologies are much rarer. Here we report a case of FFT associated with cervical carotid artery dissection that was successfully treated by surgical thromboendarterectomy.

Procedural Risks of Carotid Intervention in 19,000 patients
Randomised controlled trials (RCTs) show that carotid endarterectomy (CEA) and stenting (CAS) reduce long-term stroke risk in symptomatic and asymptomatic patients with carotid artery stenosis. Historical RCTs may not represent contemporary practice and administrative datasets may estimate procedural risks more reliably. We studied procedural risks following carotid intervention in a novel, international administrative dataset of 18,997 patients admitted to 28 hospitals across 7 countries.

Treatment of an infected, bovine pericardial carotid patch: Excision and reconstruction with a superficial femoral arterial interposition graft
We present a case of a gram-negative bacterial infection of a bovine pericardial carotid patch. Treatment ultimately required patch explantation and reconstruction with a femoral arterial interposition graft.
