Carotid Duplex Poorly Predicts Stroke Risk During Transcatheter Aortic Valve Replacement
Patients undergoing surgical aortic valve replacement, in isolation or with concomitant coronary artery bypass grafting, have historically been screened for carotid artery disease prior to surgery. Over the past decade, transcatheter aortic valve replacement (TAVR) has incrementally become the predominant technique for the treatment of severe aortic stenosis. The relationship between internal carotid artery stenosis (ICAS) and risk of periprocedural stroke in the TAVR population is uncertain. We sought to evaluate our institution's outcomes with the TAVR procedure and the association with preoperative carotid duplex scan (CDS) results.

Unplanned Shunting Is Associated with Higher Stroke Risk After Eversion Carotid Endarterectomy
Shunting during eversion carotid endarterectomy (eCEA) may be technically challenging. Whether shunting practice patterns modify perioperative stroke risk after eCEA is unclear. We aimed to compare eCEA outcomes based on shunting practice.

Regarding an Apparently High Stroke Rate in Patients With Medically Managed Asymptomatic Carotid Stenosis at an Academic Center in the Southeastern United States
We respectfully disagree with Weiner et al and advise that it is premature to give up on current best non-invasive medical intervention (lifestyle coaching and medication) alone for patients with asymptomatic carotid stenosis (ACS).(1) Weiner et al reported an average annual ipsilateral 'stroke' rate of 2.9% (10 ipsilateral strokes, in 173 patients with>70% stenosis over 2 years). This is just a little more than for non-surgically managed patients in the Asymptomatic Carotid Atherosclerosis Study (2.3%/year in patients with 60-99% ACS) and significantly more than in the most recently published quality studies of non-invasi...

Risk factors for stroke after carotid paraganglioma surgery
The Carotid Paraganglioma Cooperative International Registry (CAPACITY) is an international registry composed of 1432 patients with carotid body tumors (CBT) from 11 centers from 4 countries. The aim of this study was to identify risk factors for patients who presented stroke after carotid paraganglioma resection.

Association of metabolic syndrome with stroke, myocardial infarction, and other postoperative complications following carotid endarterectomy: A multicenter, retrospective cohort study
This study aimed to assess the impact of MetS on stroke, myocardial infarction (MI), mortality, and other complications following carotid endarterectomy (CEA).

There is No Benefit to Universal Carotid Artery Duplex Screening before a Major Cardiac Surgical Procedure
Conclusions: The majority of postoperative strokes after cardiac surgery are not related to extracranial carotid artery disease and they are not predicted by preoperative carotid artery duplex scan screening. Consequently, universal carotid artery duplex scan screening cannot be recommended and a selective approach should be adopted.

Carotid Artery Free-Floating Thrombus Caused by Paradoxical Embolization From Greater Saphenous Vein Ascending Thrombophlebitis
We report a case of a young woman without cardiovascular risk factors who was admitted to our emergency department with listlessness and altered mental status. The clinical examination revealed right lower limb swelling. Magnetic resonance imaging and contrast-enhanced computed tomographic scans revealed a free floating thrombus of the left internal carotid artery (ICA) with a large bilateral frontal ischemic lesion. The diagnosis of a medium-sized PFO with moderate right-to-left contrast shunting was made after transesophageal echocardiography. No other cardiac sources for embolization were detected, while an ascending th...

Carotid Endarterectomy in Octogenarian: Short- and Midterm Results
Conclusion: The good immediate results and good probability of survival at 3 years after surgery conveys a real benefit of this surgery in this age group, but patients should be selected on a case by case basis. In our experience, the presence of bilateral lesions should be a significant influence criterion in the preoperative risk assessment. Other evaluation criteria, such as cardiac status, are needed to optimize the selection of asymptomatic patients in particular.

Managing Recurrent Carotid Artery Disease with Redo Carotid Endarterectomy: A 10-year Retrospective Case Series
Conclusions: In our retrospective study, the stroke and restenosis rates after RCEAs were similar to those after primary CEA. Therefore, we consider RCEA to be a viable therapeutic option in patients with carotid disease that recurs after a primary CEA.

Time Is Brain? — Surgical Revascularization of Acute Symptomatic Occlusion of the Internal Carotid Artery up to One Week
Conclusions: In patients who undergo surgery after 72 hr from symptom onset, the risk seems to outweigh the benefit.

Perioperative Outcome of Carotid Endarterectomy According to Type and Timing of Neurologic Symptoms and Computed Tomography Findings
Background: Carotid endarterectomy (CEA) in the early period after the onset of cerebral neurologic symptoms seems to be useful in the prevention of recurrent stroke while safety is still under debate. Asymptomatic carotid artery stenosis could be associated with silent cerebral ischemic events (CIEs) detectable on cerebral computed tomographic (CT) scans, but is not clear if (or how) silent cerebral lesions could influence the outcome of CEA. The aim of our work was to analyze the outcome of CEA according to the timing and type of neurologic symptoms and to evaluate the outcome of CEA in asymptomatic patients with silent ...

Cannabis-Associated Arterial Disease
Conclusion: Cannabis use is associated with arterial disease such as stroke, myocardial infarction, and limbs arteritis. It appears essential to investigate cannabis use in young patients presenting with such arterial manifestations, as outcome is closely correlated with cannabis withdrawal.
