Improving Tremor Detection and Quantification with Automated Measurements of Stroke Energy (P3.014)
Conclusions:The DCTE feature reliably and precisely quantifies stroke energy associated with tremor severity, with no additional clinician or patient burden. DCTclock’s stroke energy measurement correlates with clinician judgment and can accurately distinguish ET from HC.Disclosure: Dr. Musicus has received personal compensation for activities with Aptec Group and Digital Cognition Technologies, Inc. Conclusions:Preliminary results from this trial investigating the safety and efficacy of higher-dose onabotA for elbow and shoulder spasticity indicate a dose-related benefit. At the higher total dose of 500U onabotA, no new safety signals were observed; AEs did not appear to be dose-related.Study Supported by: Allergan plcDisclosure: Dr. Patel has received personal compensation for activities with Allergan, Merz, and Ipsen as a speaker and member of scientific advisory boards. Teleneurology beyond stroke: a component of value-based specialty care (P3.034)
Conclusions:Teleneurology aims to bring expertise to the patient and not the reverse. Initially conceived as means to improve access to acute stroke care, it can be viewed as a tool with farther reaching capabilities. Such pathways are becoming increasingly common across the country, whereby our broad experience may assist prospective programs in increasing access to expertise in a safe, effective, and patient-centered and value-based approach to neurologic care.Disclosure: Dr. Cohen has received personal compensation for activities with EM Gladiators LLC. as a Medical app developer and Decibel Therapeutics as a consultant...

Stroke Code De-escalation: Safety and Outcomes (P3.035)
Conclusions:The introduction of DSCs resulted in a statistically significant absolute improvement in SCTR of 7.1%. Importantly, DSCs did not result in any eligible AIS patient forgoing IV tPA or IAT, nor missed ICH. More research is needed to increase the yield of stroke codes, refine the criteria for both activating and de-escalating them, and quantify the resource and cost implications of such de-escalations.Disclosure: Dr. Jarnot has nothing to disclose. Exercise for Brain Health. The Science of Participation in Exercise: Conceptual Framework and Practical Application (P3.036)
Conclusions:The EAST approach focuses on drivers of behavior that health systems often neglect. Onsite Consultation by Neurologist vs. Tele-Neurology, Referring Physician Satisfaction. (P3.038)
Conclusions:Onsite consultation is more preferred by providers and patients than tele-neurology consultation. The latter, neither has clinical evidence to support non-inferiority nor has popularity to grant its consideration in standard of care neurology.Disclosure: Dr. Alhatou has nothing to disclose. Percutaneous Endoscopic Gastrostomy in Patients with Acute Neurologic Injury (P3.040)
Conclusions:In the acute neurologically injured patient. PEG tubes were associated with a 19% complication rate, with an 8% risk of severe complications. Clinicians recommending PEGs in neurologically ill patients should be aware of the potentially high rates of complications.Disclosure: Dr. Vu has nothing to disclose. Impact of Patients Income on Cost and Charge of Intracranial Hemorrhage and Cerebral Infarction Hospitalization (P3.043)
Conclusions:Overall, during study period, hospital charge and cost of ICH/CI were higher in NL-group than L-group with the cost-to-charge ratio decreasing in both groups. Although healthcare policy makers and managers will find this information beneficial for further planning, dedicated studies to investigate the etiology of differences and their relations to clinical outcomes are required.Disclosure: Dr. Elliott has nothing to disclose. Early Improvement in NIH Stoke Scale during Inter-hospital Transfer is Common and is Associated with Lower In-hospital Mortality (P3.044)
Conclusions:Early improvement in stroke severity during interhospital transfer is common, and is not confined to patients receiving thrombolysis. These early improvement patients have very low mortality, and it is substantially lower than in those without early improvement, despite higher initial NIHSS. This has important implications for assessing the safety and efficacy of drip and ship thrombolysis or other pre-transfer interventions. Further research is warranted.Disclosure: Dr. Mahmood has nothing to disclose. Dr. Ali has nothing to disclose. Dr. Siddiqui has nothing to disclose. Dr. Cohen has nothing to disclose. Dr....

Assessment of Economic Aspects of Stroke in Different Clinical Management Settings: Academic vs. Non-academic (P3.055)
Conclusions:The academic status of healthcare facilities plays a key role in cost and charge of the management of stroke patients. Despite more employment of laboratory, imaging and resources in academic setting, comparison of prognosis, patient safety and clinical outcome in two groups remains unclear. Significant decrease in the cost-to-charge ratio may cause higher patients’ copay which reflects more in academic settings and uninsured individuals. Further studies to investigate effects of alterations in the cost-to-charge ratio on patients’ beneficiary is necessary.Disclosure: Dr. Fahimi has nothing to discl...

Association between frequency of sleep medication use and brain morphology (P3.058)
Conclusions:This is the first study to examine the association between frequency of sleep medication use and brain morphology in a healthy group of older adults. Moyamoya Syndrome Secondary to Vasculopathy in Down Syndrome: A Case Report and Literature Review (P3.071)
Conclusions:Our case suggests the angiographic pattern in Moyamoya syndrome due to Down syndrome vasculopathy may begin in the middle cerebral artery as it occasionally can in idiopathic Moyamoya disease with sparing of the supraclinoid internal carotid artery. Further studies are warranted to compare the angiographic and clinical course of Moyamoya syndrome due to Down syndrome vasculopathy with that of idiopathic Moyamoya disease.Disclosure: Dr. Shehabeldin has nothing to disclose. Man-in-the-barrel syndrome with Delayed Diffusion-weighted MRI Findings (P3.072)
Conclusions:DWI has a high degree of sensitivity and specificity. Nevertheless, false negative results often occur because of small infarct size, early imaging, and brainstem locations. The rate of false negative results with DWI has been estimated to be 17%. We argue it may be advisable to repeat MRI in certain cases were stroke is highly suspected up to 72 hours post symptoms. Often false negative DWI occur in posterior circulation strokes making our case unique, given it is in the anterior circulation. One possible explanation for this, is differences in cerebral reserve between a young patient versus an older patient d...
