The Impact of Prior Antithrombotic Status on Cerebral Infarction in Patients with Atrial Fibrillation
Background: Anticoagulation effectively prevents cardioembolic stroke in atrial fibrillation (AF) patients, whereas it is less effective than antiplatelet therapy (AT) in noncardioembolic stroke prevention. We hypothesized that the ischemic lesion pattern and vascular patency would differ according to the antithrombotic treatment status in AF patients. Methods: The medical records of 1078 acute ischemic stroke patients with AF were retrospectively reviewed. Patients were classified according to medication at stroke onset: (1) optimal anticoagulation (OAC; international normalized ratio [INR] 1.7-3.0; n = 36); (2) suboptim...

Diagnostic Accuracy and Risk Factors of the Different Lacunar Syndromes

Can Early Effective Anticoagulation Prevent New Lesions on Magnetic Resonance Imaging in Acute Cardioembolic Stroke?
Background: The timing of warfarin administration for acute ischemic stroke (AIS) patients with atrial fibrillation (Af) has not been established. We hypothesized that achieving targeted prothrombin time and international normalized ratio (PT-INR) at 2 weeks could prevent AIS patients with Af from developing a new lesion on diffusion-weighted magnetic resonance imaging (DW-MRI).Methods: Of consecutively enrolled AIS patients with Af between 2008 and 2011, we selected the patients who were given warfarin within 2 weeks of admission and had DW-MRI and blood test for PT-INR both on admission and at 2 weeks. Warfarin was st...

Prevalence and Clinical Correlation of Left Ventricular Systolic Dysfunction in African Americans with Ischemic Stroke
Conclusions: Prevalence of LVSD in AA with IS was 22% in our study. Smoking was a significant modifiable RF associated with systolic dysfunction. A history of smoking and higher age could predict the occurrence of LVSD. There were more large-vessel IS in patients with LVSD.

Reduced Smoke-like Echo and Resolved Thrombus in the Left Atrium with Rivaroxaban Therapy in an Acute Cardioembolic Stroke Patient
We report a case of a nonvalvular atrial fibrillation (NVAF) patient with acute cardioembolic stroke in whom rivaroxaban, an oral direct factor Xa inhibitor, reduced a smoke-like echo in the left atrium and resolved a thrombus in the left atrial appendage. A 71-year-old man was admitted because of the sudden onset of right hemiplegia and aphasia and was diagnosed with acute cardioembolic stroke associated with NVAF. The patient had not been treated with warfarin before admission, and rivaroxaban therapy (15 mg once daily) was initiated. Transesophageal echocardiography was performed on day 8 and a mobile thrombus was foun...

Cerebral Microbleeds and Asymptomatic Cerebral Infarctions in Patients with Atrial Fibrillation
Conclusions: Patients with AF had a significantly higher prevalence of CMBs. The presence of CMBs in the baseline MRI may predict the consequent onset of an ACI and increase in CMBs in patients with AF.

Prevalence of Intracranial Stenosis in a Norwegian Ischemic Stroke Population
Conclusions: IS occurs in approximately 10% and is symptomatic in about 7% of a Norwegian ischemic stroke/TIA population. Diabetes mellitus appears to be the major risk factor for IS.

Predictive Factors for Early Clinical Improvement after Intra-arterial Thrombolytic Therapy in Acute Ischemic Stroke
This study aimed to identify clinical and radiological variables that are predictive of early improvement (EI) after IAT in acute ischemic stroke. Methods: This single-center retrospective cohort study included 141 consecutive patients who underwent IAT for terminal internal carotid and/or middle cerebral artery (MCA) occlusions. EI was defined as a National Institutes of Health Stroke Scale (NIHSS) score less than 3 or NIHSS score improvement of 8 points or more within 72 hours of IAT. The EI and non-EI groups were compared in terms of clinical and radiological findings before and after IAT.Results: Forty-nine patients sh...

Efficacy of NeuroFlo Device in Treatment of Patients with Atrial Fibrillation
We report subgroup analyses of outcomes in SENTIS patients with a history or new diagnosis of AF. Among patients with AF, those treated with NeuroFlo demonstrated significant improvement over those not treated for multiple end points: global efficacy end point (P = .030), modified Rankin Scale (mRS) score 0-2 versus 3-6 (P = .029), and stroke-related mortality (P = .015). There was a significant improvement in global end point for those aged 60 years or older (P = .042) and 80 years or older (P = .017), with a trend toward improvement for age 70 years or older (P = .055), and significant improvement in those wh...

Adenosine Diphosphate–Induced Platelet Aggregation Might Contribute to Poor Outcomes in Atrial Fibrillation–Related Ischemic Stroke
Systemic atherosclerosis is involved in ischemic damages and cardioembolism after atrial fibrillation (AF)–related ischemic stroke (IS). Platelet activation is a critical factor in systemic atherosclerosis; however, there is little information regarding the role of platelet activation on the outcome of AF-related IS. We investigated the relationship between adenosine diphosphate (ADP)–induced platelet aggregation and the long-term outcomes of AF-related IS. We studied 249 patients who were exclusively treated with anticoagulation therapy after they had experienced AF-related IS. We evaluated their platelet function 5 ...
