Preprocedural Imaging
AbstractBackgroundEndovascular treatment (EVT) has strong evidence for its effectiveness in treatment of acute ischemic stroke (AIS); however, up to half of the patients who undergo EVT still do not have good functional outcomes. Various prethrombectomy radiological factors have been shown to be associated with good clinical outcomes and may be the key to better functional outcomes, reduced complications, and reduced mortality. In this paper, we reviewed the current literature on these imaging parameters so they can be employed to better estimate the probability of procedural success, therefore allowing for more effective ...

Patient outcomes after treatment of brain aneurysm in small diameter vessels with the silk vista baby flow diverter: A systematic review
CONCLUSION: Our review suggests that SVB is a safe and effective treatment for intracranial aneurysms in small vessels. Further prospective and comparative studies with patient outcome data specific to aneurysm location are needed to confirm the safety and efficacy of SVB.PMID:35392703 | DOI:10.1177/15910199221091645

Antithrombotic Treatment after Carotid Stenting in Patients with Concomitant Atrial Fibrillation INTERVENTIONAL
CONCLUSIONS: Among patients with carotid artery stent placement with atrial fibrillation, triple therapy confers a high bleeding risk. A regimen of direct oral anticoagulants plus a P2Y12 inhibitor might confer a good safety profile with significantly lower rates of bleeding and optimal efficacy.

An In-Depth Analysis of Brain and Spine Neuroimaging in Children with Abusive Head Trauma: Beyond the Classic Imaging Findings PEDIATRICS
CONCLUSIONS: Our results may suggest that a combination of intraventricular hemorrhage, diffuse axonal injury, hypoxic-ischemic injury, arterial stroke, and/or spinal ligamentous injury on neuroimaging at presentation may be used as potential poor prognostic indicators in children with abusive head trauma.

Acute carotid stenting plus aspirin during thrombectomy of tandem occlusions: A matched case-control study
CONCLUSIONS: Acute CAS plus Aspirin during thrombectomy for tandem occlusion stroke appears safe. Randomized trials are warranted to confirm these findings.PMID:37229632 | DOI:10.1177/15910199231175375

Multicenter comparison of antiplatelet treatment strategies for urgent/emergent neuroendovascular stenting
CONCLUSION: The optimal timing of antiplatelet administration in relation to stent placement and route of administration of antiplatelet agents is unclear. Timing and route of administration of antiplatelet agents may have an effect on thrombosis in emergent neuroendovascular stenting. Significant practice variation exists in antiplatelet agent utilization in emergent neuroendovascular stenting.PMID:37402388 | DOI:10.1177/15910199231180003

Treatment of an Acute Common Carotid Artery Occlusion Using a Large-Bore Aspiration Thrombectomy Device
An 86-year-old man presented with acute ischemic stroke in the left middle cerebral artery territory due to complete occlusion of the left common carotid artery (CCA) extending into the left internal and external carotid artery up to the carotid terminus (C7) (Fig 1). The etiology was surmised to be cardioembolic, in the setting of atrial fibrillation with subtherapeutic anticoagulation (international normalized ratio, 1.8). After presentation, the National Institutes of Health Stroke Scale (NIHSS) score increased from 4 (baseline) to 8 points; mechanical recanalization by percutaneous thrombus aspiration was planned.

Thunderclap headache: Diagnostic considerations and neuroimaging features
Thunderclap headache (TCH) is an acute and severe headache that has maximum intensity at onset; TCH can be primary or secondary. Primary TCH is diagnosed when no underlying cause is discovered; however, imaging is crucial in distinguishing secondary causes, which are wide-ranging. The radiologist should be aware of the list of potential diagnoses. Subarachnoid haemorrhage (SAH) is the most common cause of secondary TCH. Aneurysmal SAH accounts for the majority of cases, although other causes should also be considered and these include perimesencephalic haemorrhage, arteriovenous malformations, and dural arteriovenous fistu...

Detection of Left Atrial Appendage Thrombus by Cardiac Computed Tomography in Patients With Atrial Fibrillation: A Meta-Analysis Original Articles
Conclusions— Cardiac computed tomography, particularly when delayed imaging is performed, is a reliable alternative to TEE for the detection of LA/LAA thrombi/clot, avoiding the discomfort and risks associated with TEE.

Emergency Noninvasive Angiography for Acute Intracerebral Hemorrhage REVIEW ARTICLES
SUMMARY: Spontaneous ICH is a devastating condition and is associated with significant mortality in the acute phase due to ongoing hemorrhage and hematoma expansion. A growing body of evidence suggests that there may be considerable utility in performing noninvasive vascular imaging during the acute-to-early phase of ICH. CTA has become widely available and is sensitive and specific for detecting vascular causes of secondary ICH such as aneurysms, arteriovenous malformations, dural arteriovenous fistulas, intracranial dissections, and neoplasm. CT venography can also diagnose dural sinus thrombosis presenting as hemorrhagi...

Antithrombotic Regimens in Patients With Atrial Fibrillation and Coronary Disease Optimizing Efficacy and Safety ∗
Clear evidence supports the value of oral anticoagulation (OAC) with vitamin K antagonists in preventing stroke and thromboembolism in patients with atrial fibrillation (AF) who have well-established risk factors. For this indication, vitamin K antagonists have been shown to be superior to single or dual antiplatelet agents in reducing thromboembolic complications (1). Yet, up to 30% of patients with AF also have indications for antiplatelet therapy because of coronary artery disease (2). Dual antiplatelet therapy with aspirin and a P2Y12 receptor inhibitor (e.g., clopidogrel) is usually recommended after stent implantatio...

4D CT Angiography More Closely Defines Intracranial Thrombus Burden Than Single-Phase CT Angiography BRAIN
CONCLUSIONS: As the result of an increased sensitivity for collateral flow, 4D-CTA temporal maximum intensity projection more closely outlines intracranial thrombi than conventional single-phase CT angiography. Our findings can be helpful when planning acute neurointervention. Further research is necessary to validate our data and assess the use of 4D-CTA in predicting response to different recanalization strategies.
