Correlation of imaging and histopathology of thrombi in acute ischemic stroke with etiology and outcome: a systematic review
Conclusions The hyperdense artery sign is associated with RBC-rich thrombi and improved recanalization rates. However, there was no association between the histopathological characteristics of thrombi and stroke etiology and angiographic outcomes.

Revisiting the NIH Stroke Scale as a screening tool for proximal vessel occlusion: can advanced imaging be targeted in acute stroke?
Conclusions In this cohort, specific NIHSS criteria were highly sensitive for emergent large vessel occlusion and, if validated, may allow for clinical screening prior to advanced imaging with CT angiography.

Carotid I's, L's and T's: collaterals shape the outcome of intracranial carotid occlusion in acute ischemic stroke
Conclusions Collateral flow patterns distinguish the nature and impact of ICA occlusions on expected revascularization and subsequent clinical outcomes in acute ischemic stroke. The nomenclature of terminal ICA occlusions introduced here (carotid I's, L's, and T's) may enhance future endovascular trials targeting such proximal occlusions. Trial registration number NCT00318071 (http://clinicaltrials.gov). MERCI was not registered because enrollment began before July 1, 2005.

Mechanical thrombectomy versus systemic thrombolysis in MCA stroke: a distance to thrombus-based outcome analysis
Conclusions In patients with an acute MCA occlusion and a DT <16 mm, IVT–MT leads to a significantly better outcome than in patients treated with IVT alone.

Impact of thrombus length on recanalization and clinical outcome following mechanical thrombectomy in acute ischemic stroke
Conclusions In this study, thrombus length had no relevant impact on recanalization, neurological outcome, or intraprocedural complications following mechanical thrombectomy of middle cerebral artery occlusions. Therefore, mechanical thrombectomy with stent retrievers can be attempted with large clots.

O-012 Relationship between clot composition and procedural results for ischemic stroke aspiration thrombectomy: subset analysis from the INSIGHT registry
ConclusionsClot removed during neuroendovascular procedures can differ in consistency and removability, leading to an increased focus on thrombus composition as a crucial variable for improving outcomes of ischemic stroke. In this study, good final revascularization (mTICI 2b-3 at the final angiogram) occurred after aspiration thrombectomy regardless of clot composition. Ongoing analysis of collected specimens is expected to provide valuable information regarding thrombus biology and etiology and to aid in diagnosis, prognosis, and drug development.Abstract O-012 Table 1Clot composition vs procedural results for ischemic s...

O-012 Emergent Endovascular Management of Long-segment Carotid Artery Dissections in Acute Ischemic Stroke Intervention with Multiple Tandem Stents
ConclusionTandem stent reconstruction for the treatment of long segment and flow limiting carotid dissections is technically safe and effective with favorable clinical outcomes in acute ischemic stroke intervention, allowing for successful thrombectomy, vessel salvage, restoration of cerebral perfusion, and prevention of recurrent thromboembolic stroke.DisclosuresS. E-004 Factors Associated with Successful Revascularization using the Aspiration Component of ADAPT in the Treatment of Acute Ischemic Stroke
ConclusionsAspiration success was associated with younger age. There was a trend for aspiration failure to be associated with cardiogenic etiology and more difficult vascular anatomy. Our findings suggest that the ADAPT technique can be used for the vast majority of patients but it may be beneficial to use a SR primarily when performing thrombectomy in the elderly. Further research analyzing thrombus characteristics is needed.References1 Turk AS, Frei D, Fiorella D, et al. ADAPT FAST study: a direct aspiration first pass technique for acute stroke thrombectomy. E-189 Outcomes of tenecteplase versus alteplase after mechanical thrombectomy in a multiethnic stroke consortium registry
ConclusionIn this prospective, multi-center stroke consortium study, IV TNK in comparison to tPA prior to MT for ischemic stroke resulted in non-inferior outcomes related to angiographic reperfusion and functional status at discharge. These findings compliment the current literature and include a large Hispanic US cohort. Further analysis will include expansion of contributing centers and use of propensity scoring.Abstract E-189 Table 1 and 2Disclosures M. Gaub: None. D. Ramaswamy: None. A. Cardentey: None. S. 64-detector CT angiography within 24 hours after carotid endarterectomy and correlation with postoperative stroke.
CONCLUSIONS CTA within 24 hours of CEA demonstrates characteristic anatomical findings. CCA step-offs and ECA flaps are relatively common and clinically insignificant, whereas ICA/CCA flaps and thrombi are less frequently seen and are associated with postoperative stroke/TIA.
