ConclusionsThis study points, for the first time, to differences in the proteomic landscapes between thrombi from AIS patients that are related to time since symptom onset. Taken together, the present data support a view of the stroke thrombus as a living, dynamic microenvironment with increasing intracellular protein and RNA processing with increased tLKW. E-166 The effect of covid-19 on treatment and outcomes following ischemic stroke: a national assessment
ConclusionThis study identified a relationship between COVID-19 diagnosis and worse outcomes for each metric assessed. Most notably, even when controlling for disease-related factors, patients with COVID-19 were significantly less likely to undergo mechanical thrombectomy. Further, COVID-19 was associated with increased in-hospital mortality and worsened discharge disposition, although the exact causal relationship of this is unclear. Delayed ischemic stroke following spontaneous thrombosis of an arteriovenous malformation
We present a unique case of a middle-aged patient with spontaneous obliteration of a cerebral arteriovenous malformation (AVM) who developed an ischemic stroke due to thrombosis of the stagnant proximal segment of the inferior branch of the middle cerebral artery feeder. Although the pathophysiology is not well understood, the arterial feeder hemodynamic changes post SOAVM may behave similarly to what occurs in rare cases after surgical resection of AVMs. Our case raises the hypothesis that stagnation of flow in spontaneous AVM obliteration may lead to delayed ischemic stroke in the territory of the feeding artery.

O-006 Analysis of Screened Patients from the Penumbra THERAPY Trial: Correlations of Clot Length Assessed by Thin-Section CT in a Sequential Series of Acute Stroke Patients
Conclusion This analysis further supports the idea of measuring hyperdense thrombus by use of thin-section NCCT. Occlusion site is a key predictor of clot length, which may explain the relative ineffectiveness of IV rtPA in more proximal anterior circulation large vessels. E-031 Treatment of Acute Ischemic Stroke Due to Thrombosis of Stenotic Atherosclerotic Lesions of the Posterior Circulation
Conclusion Endovascular treatment can be performed with high rates of technical success for AIS due to thrombosis at chronic atherosclerotic lesions. In spite of potential for poor outcomes, significant recovery of function is possible following treatment. P-015 flow stasis after the deployment of stent-retriever predicts successful final recanalization in acute stroke patients
ConclusionLoss of IFR effect during the embedding time after deployment of stent retriever was associated with successful recanalization. It may be an angiographic sign of adequate thrombus engagement with the stent retriever. E-071 initial experiences with the 5max ace 64 in the endovascular management of acute ischemic stroke
ConclusionIn our experience, this new version of the 5Max is very efficient in the aspiration of the thrombus. Due to its larger lumen, it’s more difficult to navigate over the smaller microcatheters through tortuous segments of the target vessels, especially the ophthalmic region of the ICA. Using the 3Max instead, the navigation gets easy, but it’s very difficult to get a roadmap or perform an angiogram through the ACE, due to the larger OD of the 3Max. Emergency Superficial Temporal Artery to Middle Cerebral Artery Bypass After Intravenous Administration of Tissue Plasminogen Activator for Stroke.
We described three cases of emergency superficial temporal artery to middle cerebral artery bypass performed within 24 hours of IVtPA administration, and discussed the safety of such procedures. Successful outcome after endovascular thrombolysis for acute ischemic stroke with basis on perfusion-diffusion mismatch after 24 h of symptoms onset.
CONCLUSIONS: We report the case of a successful thrombolytic therapy following AIS performed more than 24 h after the initial symptoms based on the presence of a perfusion-diffusion mismatch. This report is expected to stimulate the development of future prospective studies with special focus on the role of perfusion-diffusion mismatch in patient selection for treatment of AIS, especially in those presenting outside the traditional time window. Management and outcome of spontaneous cerebral venous sinus thrombosis in a 5-year consecutive single-institution cohort
Conclusions In our series, the majority (92.9%) of patients with spontaneous dural sinus thrombosis had a favorable clinical outcome as defined by a mRS ≤2. Further prospective studies are needed to study the impact of anticoagulation on the clinical course of the disease.

Radiology ruse: dural venous sinus cyst masquerading as venous sinus thrombosis presenting with headache and stroke
Conclusions Radiological differentiation between venous sinus thrombosis and dural venous sinus cyst is clinically important to avoid unnecessary anticoagulation and associated risks. Final management included propranolol for migraine prophylaxis; aspirin for acute migraine; aspirin for stroke prevention; vascular risk factor management; and cessation of triptans.
