Conclusions: Patients on long-term support have an excellent haemodynamic profile and show an acceptable risk-benefit ratio. Although heart transplantation remains the ultimate target at present, our data show that long-term circulatory support is a viable option with the potential to become a more permanent solution in the long-term.

Thromboembolism in a patient with a mechanical mitral valve during anticoagulation with dabigatran etexilate.

Ten-year screening for thrombocytopenia after aortic valve replacement
CONCLUSIONS Thrombocytopenia and Elisa (+) are more common after AVR than after other procedures, and both were associated with increased adverse clinical outcomes. Age and lower preoperative platelet count were associated with postoperative thrombocytopenia and Elisa (+).

Mechanical heart valve prosthesis and warfarin – Treatment quality and prognosis
Conclusion: Patients with a lower warfarin treatment quality measured by TTR have a higher risk of complications such as severe bleeding or death. A TTR of 83% or higher at the individual level should be obtained for best outcome.

Efficacy and safety of early parenteral anticoagulation as a bridge to warfarin after mechanical valve replacement.
In conclusion, we found that early after mechanical valve replacement, therapeutic dose bridging was associated with a similar risk of thromboembolic complications, but a 2.5 to 3-fold increased risk of major bleeding compared with prophylactic dose bridging. 013 * european multicentre experience with sutureless perceval valve: clinical and haemodynamic outcomes up to 5 years in over 700 patients
Conclusion: This European multicentre experience with the largest cohort of patients with sutureless valves to date shows excellent clinical and haemodynamic results that remain stable even up to 5 years follow-up. Even in this elderly patient cohort with 40% octogenarians, both early and late mortality were very low. There were no valve migrations, structural valve degeneration or valve thrombosis in follow-up. The sutureless technique is a promising alternative to biological AVR.

Conclusion: The calculated annual failure rate after VSRR using the reimplantation technique was lower than the annual risk for thromboembolic or bleeding events. As the perioperative risk of reinterventions after VSRR itself is low, patients might benefit from VSRR even if they have to undergo redo surgery during follow-up.

Pontine infarction responsible for wall-eyed bilateral internuclear ophthalmoplegia syndrome
A 68-year-old man with hypertension, cardiac arrhythmia, and a mechanical heart valve replacement treated with vitamin K antagonists was admitted for the acute onset of diplopia. Neuro-ophthalmologic examination showed a bilateral exotropia in primary gaze position (wall-eyed) with bilateral adduction deficit that did not improve by convergence (figure, video at Neurology.org/cp).
