The effect of butylphthalide on improving the neurological function of patients with acute anterior circulation cerebral infarction after mechanical thrombectomy
Medicine (Baltimore). 2023 Aug 25;102(34):e34616. doi: 10.1097/MD.0000000000034616.ABSTRACTButylphthalide can improve blood circulation in patients with acute cerebral infarction. Complement 3a receptor 1 (C3aR1) is involved in the regulation of innate immune response and pathogen monitoring, which is closely related to the pathophysiological processes of breast cancer, neurogenesis and lipid catabolism. Our study explored the therapeutic effect of butylphthalide on improving the neurological function of patients with acute anterior circulation cerebral infarction after mechanical thrombectomy, and evaluated the correlatio...

Treatment of an Acute Common Carotid Artery Occlusion Using a Large-Bore Aspiration Thrombectomy Device
An 86-year-old man presented with acute ischemic stroke in the left middle cerebral artery territory due to complete occlusion of the left common carotid artery (CCA) extending into the left internal and external carotid artery up to the carotid terminus (C7) (Fig 1). The etiology was surmised to be cardioembolic, in the setting of atrial fibrillation with subtherapeutic anticoagulation (international normalized ratio, 1.8). After presentation, the National Institutes of Health Stroke Scale (NIHSS) score increased from 4 (baseline) to 8 points; mechanical recanalization by percutaneous thrombus aspiration was planned.

The potential role of micro-RNA 125b-5p level in predicting outcome from thrombolytic therapy in patients with acute ischemic stroke
AbstractSeveral studies highlighted a significant role of specific miRNA as diagnostic and prognostic biomarkers for acute ischemic stroke. The aim of this work was to study micro-RNA 125b-5p level in patients with acute ischemic stroke in relation to stroke etiology, risk factors, severity and outcome. This case –control study was conducted on 40 patients with acute ischemic stroke eligible for receiving rt-PA and 40 age and sex matched healthy controls, Patients were submitted to neurological and radiological assessment. Functional outcome after 3 months was assessed using the modified Rankin Scale (mRS ). Plasma micr...

Prediction of blood pressure variability during thrombectomy using supervised machine learning and outcomes of patients with ischemic stroke from large vessel occlusion
AbstractMechanical thrombectomy (MT) is the standard of care for patients with acute ischemic stroke from large vessel occlusion (AIS-LVO). The association of blood pressure variability (BPV) during MT and outcomes are unknown. We leveraged a supervised machine learning algorithm to predict patient characteristics that are associated with BPV indices. We performed a retrospective review of our comprehensive stroke center ’s registry of all adult patients undergoing MT between 01/01/2016 and 12/31/2019. The primary outcome was poor functional independence, defined as 90-day modified Rankin Scale (mRS) ≥ 3. We used p...

Predictors and long-term outcome of intracranial hemorrhage after thrombolytic therapy for acute ischemic stroke —A prospective single-center study
DiscussionOlder age, higher NIHSS, large vessel occlusion, and intra-arterial thrombolysis may correlate with ICH. The unfavorable outcome is more common in patients with ICH. Precise scoring of post-thrombolysis bleeding might be a useful tool in the evaluation of the patient's prognosis. Our findings may help to identify predictors and estimate the prognosis of ICH in patients with AIS treated with rt-PA.

Mechanical Thrombectomy for Acute Common Carotid Artery Occlusion
Neurol Med Chir (Tokyo). 2023 Jan 5. doi: 10.2176/jns-nmc.2022-0183. Online ahead of print.ABSTRACTMechanical thrombectomy (MT) is the standard treatment for acute large occlusion of the cerebral artery. Evidence for the success of this procedure was based on the treatment of patients with internal carotid artery and middle cerebral artery thrombi. There are a few reports on thrombi extending to the common carotid artery (CCA). We document our endovascular procedure and the clinical outcome in seven consecutive patients who underwent MT for CCA thrombi between September 2016 and April 2021. Their mean National Institutes o...

P74 Keep it simple: MAC (Manual Aspiration with Contrast) technique for the endovascular management of acute ischemic stroke (LVO & MVO)
We report a prospectively collected clinical experience with a simple technique: MAC (Manual Aspiration Contrast Enhancement) as the primary method for vessel recanalization.Gently contrast injection while the aspiration catheter is advancedto the thrombus and subsequentlycreating a closed-loop system with the contrast column within the cathetercan result in better visualization during the aspiration, thus improving the FPEavoiding clot fragmentation, multiple passes and blind movements of catheter by a single operator.Methods47 prospectively patients with ELVO and 3 cases of middle vessel occlusion (MVO) at four instituti...

Effect of Solitaire FR Stent Thrombectomy Combined with the Suction Thrombus on the Clinical Effect and Prognosis of Acute Middle Cerebral Artery Occlusion
Evid Based Complement Alternat Med. 2022 Jul 30;2022:9227790. doi: 10.1155/2022/9227790. eCollection 2022.ABSTRACTTo determine the curative effect and prognosis of Solitaire FR stent thrombectomy integrated with the suction thrombus on the treatment of acute middle cerebral artery occlusion (AMCAO). Based on the treatment, patients suffering from AMCAO were separated into the Solitaire FR group (Solitaire FR stent + suction thrombus) and suction group (suction thrombus). Modified thrombolysis in cerebral infarction grading, National Institutes of Health Stroke Scale (NIHSS) score, modified Rankin Scale score, and safety pe...
