Paradoxical venous air embolism detected with point-of-care ultrasound: a case report
We present a case of a critical care patient who presented sudden clinical hemodynamic deterioration after the removal of central venous catheter. Hemodynamic evaluation with point-of-care ultrasound (POCUS) showed bubbles in both right and left heart cavities wherewith air embolism facilitated by heart septal defect was suspected. Therefore, the patient was reintubated, supported with vasopressors and a new CVC was inserted to proceed with air aspiration. Shortly after, the patient ’s hemodynamic status improved in terms of vital signs stabilization. 6 h after the event with optimal perfusion markers and diminished sed...

Improved detection of coarctation of the aorta using speckle tracking analysis of the fetal heart using the last examination prior to delivery.
CONCLUSIONS: Speckle tracking analysis of multiple ventricular measurements may be helpful to separate fetuses that are suspected to have CoA prenatally from those who do not. This article is protected by copyright. All rights reserved. PMID: 32022339 [PubMed - as supplied by publisher]

Human Fetal Hearts with Tetralogy of Fallot have Altered Fluid Dynamics and Forces.
In this study, we performed patient-specific ultrasound-based flow simulations of 3 TOF and 7 normal human fetal hearts. TOF right ventricles (RV) had smaller end-diastolic volumes (EDV) but similar stroke volumes (SV), while TOF left ventricles (LV) had similar EDV but slightly increased SV compared to normal ventricles. Simulation showed that TOF ventricles had elevated systolic intra-ventricular pressure gradient (IVPG), and required additional energy for ejection, but IVPG elevations were considered to be mild relative to arterial pressure. TOF RV and LV had similar pressures due to equalization via ventricular septal ...

Recent Nationwide Trends in Transcranial Doppler Utilization During Acute Ischemic Stroke Hospitalizations (P4.296)
Conclusions:Over the last decade there has been a slight but steady decrease in the use of TCD during AIS hospitalizations in the US. Future studies should examine the cost-utility of TCD, especially with the advent of endovascular stroke treatment systems.Disclosure: Dr. Liu has nothing to disclose. Cardiovascular diseases in patients 65 years and younger with non-cardiogenic stroke.
CONCLUSIONS: Aetiologically heterogeneous stroke and cardiogenic stroke are the most commonly observed among young stroke patients. Cardiomyopathy and atrial fibrillation are the most common sources of cerebral embolism in young patients with cardiogenic stroke. Nearly 1/5 of patients with a non-cardiogenic stroke have congenital or acquired structural changes in the heart. PMID: 27279848 [PubMed]

Atrial Septal Defects and Cardioembolic Strokes
Atrial septal defects (ASDs) can be complicated by cardioembolic strokes, but the exact incidence is unknown. Patients with large and small shunts may present with a cardioembolic stroke. Patients with cryptogenic strokes should have cardiac ultrasound to see if an ASD is present. Cardioembolic strokes associated with ASD principally occur with 2 mechanisms. The first is paradoxic embolism involving a venous-based source of thrombus, which may subsequently pass through the ASD by right-to-left shunting, causing a cardioembolic stroke. The second is atrial fibrillation that can complicate the course of patients with ASDs, e...

Quantification of Shunt Volume Through Ventricular Septal Defect by Real-Time 3-D Color Doppler Echocardiography: An Study
Quantification of shunt volume is important for ventricular septal defects (VSDs). The aim of the in vitro study described here was to test the feasibility of using real-time 3-D color Doppler echocardiography (RT3-D-CDE) to quantify shunt volume through a modeled VSD. Eight porcine heart phantoms with VSDs ranging in diameter from 3 to 25 mm were studied. Each phantom was passively driven at five different stroke volumes from 30 to 70 mL and two stroke rates, 60 and 120 strokes/min. RT3-D-CDE full volumes were obtained at color Doppler volume rates of 15, 20 and 27 volumes/s.
