E-057 Process Improvement in Door-to-Groin Puncture Times at a Comprehensive Stroke Center Shows a Trend Towards Reduced Mortality at 90 Days
ConclusionOur systematic process improvement initiative significantly reduced door-to-groin puncture times and showed a trend towards reduced mortality at 90 days. A multidisciplinary approach and ensuring hospital system investment are key to an effective process.DisclosuresY. Kayan: 2; C; Medtronic, Penumbra. J. Delgado Almandoz: 2; C; Medtronic, Penumbra. M. I Never Thought Stroke Would Happen to Me
by Myra Wilson, Stroke Survivor On November 3, 2014, I was in nursing school working as a student nurse at a hospital in Seattle. My first sign that something was not quite right was when I was walking through the nursing station and both of my eyes went blurry. I could still see color but I couldn't see letters. It was blurry for about 30 seconds before clearing up again. I was going to lunch and went to give a report to another nurse. The nurse noticed while I was speaking that I slurred my speech. I didn't notice my speech was slurred at all. It was at that time that I experienced a sudden sharp pain on the right s...

Safety and Efficacy of IV-tPA Use in Patients 80 Years and Older- A Retrospective Review (P2.305)
Conclusion—We observed no increase in sICH post IV-tPA in patients 80+ when compared to those younger. Higher mortality in those 80+ likely due to baseline disabilities. Within the 80+ group, predictors of mortality and poor outcomes included disabilities prior to hospitalization, prior stroke/TIA, higher NIHSS at admission, sICH post treatment and higher diastolic blood pressure.Disclosure: Dr. Lau has nothing to disclose. The role of copeptin in patients with subarachnoid haemorrage.
CONCLUSION: Patients with a gradual increase of copeptin values showed to have bad prognosis of the disease. Four with extremely high copeptin values passed away, while patients with a gradual decrease or a normal amount of copeptin values had good prognosis. It is supported that copeptin values are a reliable prognostic factor in monitoring patients with intracranial haemorrhage. PMID: 26665233 [PubMed - as supplied by publisher]

Pre-hospital emergency skills for medical students
Conclusion These results show that there is some anxiety amongst students about being able to manage pre-hospital emergencies in which the public might expect them to cope or in which they feel obliged to help. The results show that although students would be willing to help they may not posess the necessary knowledge to do so. They show that there is a demand for teaching focussed on these skills but that there is not necessarily a need for it taking into account the frequency of incidents encountered. A trial teaching programme will take place in July.

Cerebral vasospasm patterns following aneurysmal subarachnoid hemorrhage: an angiographic study comparing coils with clips
Conclusions Patients with aSAH treated by endovascular coiling and surgical clipping demonstrate distinct vasospasm patterns. While both initially exhibit perianeurysmal spasm, patients treated by coiling go on to develop stepwise progression distally over time. This finding may reflect dispersion of blood breakdown products along preserved CSF egress pathways in patients treated by endovascular coiling.

Neurologic Complications of Extracorporeal Membrane Oxygenation.
CONCLUSIONS: Given the increasing utilization of ECMO and the high costs and poor outcomes associated with neurologic complications, more research is needed to help determine the best way to prevent these sequelae in this patient population. PMID: 26320848 [PubMed - as supplied by publisher]

Significance of Periclot MMP-9 in Ischemic Stroke Patients Undergoing Intra-arterial Interventions (P4.303)
CONCLUSIONS: Decreased periclot MMP-9 may be predictive of HT after ischemic stroke, contrasting with previous work demonstrating elevated peripheral MMP-9 as predictive of HT. Increased periclot MCP-4 and angiogenin expression, as well as decreased numbers of circulating monocytes, may serve as additional predictive markers of HT. Future work should measure the activity of periclot MMP-9. Study Supported by: Shimojani, LLC and NIH (DJ).Disclosure: Dr. Song has nothing to disclose. Dr. Prager has nothing to disclose. A Novel Method for Measuring Oxidative Stress in Patients with Stroke Symptoms (S52.005)
CONCLUSIONS: These results demonstrate the ability of ORP to identify oxidative stress and amount of antioxidant reserves in a stroke population. We propose ORP monitoring as a potentially useful tool in evaluation of acute stroke patients.Disclosure: Dr. Wagner has received personal compensation for activities with Genentech, Inc. Resting Energy Expenditure in Patients with Stroke during the Subacute Phases - Relationships with Stroke Types, Location, Severity of Paresis, and Activities of Daily Living
Background: The energy demands in patients with stroke during the subacute phases are unclear. However, this information is essential for appropriate clinical and nutritional management. The aims of this study were to determine the resting energy expenditure (REE) during the subacute phases, examine its relationships with stroke types, location, severity of hemiparesis, and activities of daily living (ADL), and evaluate whether estimation of REE from the Harris-Benedict equation (HB) requires the addition of a ‘stress factor' to capture possible additional REE imposed by stroke. Methods: We measured REE in 95 patients wi...

Outcomes of endovascular mechanical thrombectomy and intravenous tissue plasminogen activator for the treatment of vertebrobasilar stroke.
CONCLUSIONS: Patients requiring IV-tPA and/or mechanical thrombectomy for the treatment of posterior-circulation strokes suffer from high mortality rates. Increased age is associated with significantly higher mortality rates among posterior-circulation stroke patients who require mechanical thrombectomy. PMID: 24465258 [PubMed]

Glucose and lipid assessment in patients with acute stroke
Conclusions: The detection of abnormal metabolic milieu is a window of opportunity for aggressive management in persons with stroke as this will improve outcome. Routine screening for hyperglycaemia in persons with stroke using glycosylated haemoglobin tests and blood glucose may uncover previously undiagnosed DM.

P-014 Improvement in Angiographic Transit Times Post Endovascular Vasospasm Treatment in Patients with Aneurysmal Subarachnoid Haemorrhage
Conclusion The DSA TT correlated with improvements in neurological status in patients with clinical vasospasm. TT might reflect a better way to assess vasospasm severity and response to treatment as compared to vessel diameter. This method may serve as a useful indirect technique for cerebral blood flow assessment in the angiography suite.
