The changes of secondary prevention practice in Czech post-stroke patients between 2007 and 2012/13
In conclusion, despite substantial improvement in acute management, clinical practice in secondary prevention in post-stroke patients remains far from being optimal.

Recurrent Ischemic Stroke: Strategies for Prevention.
Abstract Recurrent strokes make up almost 25% of the nearly 800,000 strokes that occur annually in the United States. Risk factors for ischemic stroke include hypertension, diabetes mellitus, hyperlipidemia, sleep apnea, and obesity. Lifestyle modifications, including tobacco cessation, decreased alcohol use, and increased physical activity, are also important in the management of patients with a history of stroke or transient ischemic attack. Antiplatelet therapy is recommended to reduce the risk of recurrent ischemic stroke. The selection of antiplatelet therapy should be based on timing, safety, effectiveness, ...

Combined impact of body mass index and glycemic control on the efficacy of clopidogrel-aspirin therapy in patients with minor stroke or transient ischemic attack.
CONCLUSIONS: Compared with aspirin alone, efficacy of clopidogrel-aspirin therapy for stroke still exists in overweight/obesity patients with normal glycemic control. METHODS: In Clopidogrel in High-Risk Patients with Acute Nondisabling Cerebrovascular Events trial, 3044 patients with available baseline GA were recruited. Low/normal weight and overweight/obesity were defined as BMI < 25 kg/m2 and ≥ 25 kg/m2, respectively. Elevated and low GA levels were defined as GA levels > 15.5 % and ≤ 15.5 %, respectively. The primary outcome was stroke recurrence during the 90-day follow-up. PMID: 32544082 [PubMed ...

Impact of body mass index on efficacy and safety of ticagrelor versus clopidogrel in patients with minor stroke or transient ischemic attack
CMAJ. 2023 Jul 10;195(26):E897-E904. doi: 10.1503/cmaj.230262.ABSTRACTBACKGROUND: Body mass index (BMI) may affect the response to platelet P2Y12 receptor inhibitors. We aimed to explore whether BMI influenced the efficacy and safety of ticagrelor and clopidogrel for secondary prevention of minor ischemic stroke or transient ischemic attack (TIA) among patients enrolled in the CHANCE-2 (Ticagrelor or Clopidogrel with Aspirin in High-Risk Patients with Acute Nondisabling Cerebrovascular Events II) trial.METHODS: In a multicentre, randomized, double-blind, placebo-controlled trial, conducted in China, we randomized patients ...

E-004 Focal intracranial vasculopathy as a manifestation of COVID-19-associated acute ischemic stroke
ConclusionTo our knowledge, our case is the first to illustrate the potential for COVID-19 infection to present as a focal intracranial vasculopathy in an otherwise healthy young-patient, resulting in acute ischemic stroke without an underlying hypercoagulable state. Rescue intracranial stenting was necessary to maintain vessel patency and restore intracranial flow.Disclosures A. Mahapatra: None. A. Witek: None. G. Toth: None.

Palinopsia in a Patient with an Acute Occipital Lobe Stroke (P4.369)
Discussion: Palinopsia is the persistence or recurrence of visual images after the visual stimulus has been removed. Palinopsia can be further subtyped with one of the categories being scene perseveration, where a formed scene may continuously replay for a length of time either superimposed in a visual field or in place of an impaired visual field. There have been rare other cases described with right hemispheric strokes causing similar phenomena of scene perseveration. Our case is unique that in previously reported literature, it was very rare to have isolated palinopsia without hemianopsia (1 out of 32 patients in one st...

Natural History of Non-operative Management in Asymptomatic Patients with 70%-80% Internal Carotid Artery Stenosis by Duplex Criteria.
CONCLUSION: Patients with asymptomatic moderate to severe carotid stenosis had a low rate of stroke/TIA without documented progression. However, there was a high rate of stenosis progression reinforcing the need to follow these patients closely. PMID: 32660806 [PubMed - as supplied by publisher]
