Endoaortic Clamping Does Not Increase the Risk of Stroke in Minimal Access Mitral Valve Surgery: A Multicenter Experience.
CONCLUSIONS: Once procedural proficiency is acquired, endoaortic balloon clamping in MIMVS is a safe and effective technique. Despite the fact that this patient cohort also includes combined and redo procedures, the observed mortality and stroke rate compared favorably with the existing literature on primary isolated mitral valve surgery irrespective of the approach. Risk of Stroke and Major Bleeding with Vitamin-K Antagonist Use after Mitral Valve Repair
CONCLUSIONS: Our study suggests that VKA after MVr does not reduce risk of cerebral embolic events but is associated with increased risk of major bleeding events.PMID:36223805 | DOI:10.1016/j.athoracsur.2022.09.038

A More Specific Anticoagulation Regimen Is Required for Patients After the Cox-Maze Procedure.
CONCLUSIONS: Our results indicate that the decision to discontinue OAC after the Cox-Maze procedure should not be based solely on CHADS2 scores; rather, rhythm status, echocardiographic findings, and patient risk for bleeding should be considered. These findings underscore the need for an OAC protocol for patients who have undergone the Cox-Maze procedure with appropriate LAA management. PMID: 25152383 [PubMed - as supplied by publisher]

Valve-Sparing Root Replacement vs Composite Valve Grafting in Aortic Root Dilation: A Meta-Analysis.
CONCLUSIONS: Aortic valve sparing appears to be safe, and associated with reduced late mortality, thromboembolism/stroke and bleeding compared with composite valve grafting. Late durability is equivalent. Aortic valve sparing should be considered in patients with favorable aortic valve morphology. PMID: 31981499 [PubMed - as supplied by publisher]

Mid-term results after simultaneous carotid artery stenting and cardiac surgery.
CONCLUSIONS: In this small group of patients, the hybrid procedure was proved to be a safe and efficient way of treatment for patients with concomitant carotid and cardiac diseases. The low rate of perioperative complications and good midterm results are encouraging. PMID: 32247782 [PubMed - as supplied by publisher]

Long Term Outcomes and Anticoagulation in Mitral Valve Surgery - A Report from the Society of Thoracic Surgeons Database
CONCLUSIONS: Anticoagulation was used in less than half of mitral valve surgery. In MVrep patients, warfarin was associated with increased bleeding and was not protective against either stroke or mortality. In BMVR patients, warfarin was associated with a modest survival benefit, increased bleeding and equivalent stroke risk. NOAC was associated with increased adverse outcomes.PMID:37308066 | DOI:10.1016/j.athoracsur.2023.05.025

Does Surgical Ablation Energy Source Affect Long-Term Success of the Concomitant Cox Maze Procedure?
CONCLUSIONS: Concomitant CM procedures performed with cryothermal energy alone or combined with bipolar radiofrequency ablation are safe and exceedingly effective. The association of cryothermal energy alone with higher rates of sinus rhythm and stroke reduction should be investigated further. PMID: 28577848 [PubMed - as supplied by publisher]

Probability of Uneventful Recovery after Elective Aortic Root Replacement for Aortic Aneurysm.
CONCLUSIONS: This study provides novel information regarding the probability of uneventful recovery while confirming low in-hospital mortality and stroke rates after elective aortic root replacement for aortic aneurysm. Importantly, uneventful recovery ensures excellent long-term survival. PMID: 32246934 [PubMed - as supplied by publisher]

Non-Vitamin K Antagonist Oral Anticoagulant vs Warfarin for Post Cardiac Surgery Atrial Fibrillation.
CONCLUSIONS: NOACs are associated with a reduction in post-operative LOS, without excess bleeding or other short-term complications, compared to warfarin. These findings support the broader use of NOACs as a safe alternative to warfarin in patients with post-cardiac surgery AF at elevated stroke risk and acceptable bleeding risk. Prosthesis selection for aortic valve replacement with concomitant coronary artery bypass graft
CONCLUSIONS: Long-term survival, overall reoperation, or stroke incidence was comparable among the two groups, while patients with a mechanical valve showed a greater likelihood of major bleeding events. Regarding the limited durability of bioprostheses, a larger sample size followed for 15 or more years will be necessary to determine the optimal aortic valve prosthesis for patients aged 50 to 69 years undergoing concurrent AVR and CABG.PMID:33667456 | DOI:10.1016/j.athoracsur.2021.02.033

Transapical Aortic Valve Implantation in Patients With Previous Cardiac Surgery.
CONCLUSIONS: Transapical transcatheter aortic valve implantation has simplified surgical treatment of high-risk patients with previous cardiac surgery and severe aortic valve stenosis and is associated with minimal risk of stroke. Furthermore, current data suggest that the presence of previous cardiac surgery does not impair outcomes after transapical transcatheter aortic valve implantation, making this subset of patients particularly applicable for this evolving approach. Complications of Extracorporeal Membrane Oxygenation for Treatment of Cardiogenic Shock and Cardiac Arrest: A Meta-Analysis of 1,866 Adult Patients.
CONCLUSIONS: Although ECMO can improve survival of patients with advanced heart disease, there is significant associated morbidity with performance of this intervention. These findings should be incorporated in the risk-benefit analysis when initiation of ECMO for cardiogenic shock is being considered. PMID: 24210621 [PubMed - as supplied by publisher]
