Coronary Revascularization with Single Versus Bilateral Mammary Arteries: Is It Time to Change?
CONCLUSIONS: In a large regional experience BIMA is associated with no upfront risk of adverse events and improved long-term survival compared to SIMA. Our results indicate that BIMA conduits should be considered more frequently during CABG due to their demonstrated survival advantage. PMID: 29550203 [PubMed - as supplied by publisher]

Hybrid Coronary Revascularization Versus On-Pump Coronary Artery Bypass Grafting.
CONCLUSIONS: For selected patients, HCR is associated with a faster postoperative recovery as well as similar short-term and long-term outcomes when compared with standard on-pump CABG. PMID: 29571730 [PubMed - as supplied by publisher]

Valvectomy versus Replacement for the Surgical Treatment of Tricuspid Endocarditis.
CONCLUSIONS: Tricuspid valve excision, repair, and replacement have similar 30-day operative mortality, as defined by the STS, in TV endocarditis patients and significantly lower unplanned readmission rates at one year in the excision patients. Tricuspid valvectomy is an acceptable initial treatment in this high-risk group as part of a surgical strategy to identify patients who are candidates for eventual valve replacement. Further study of long term outcomes and survival is warranted. PMID: 29777672 [PubMed - as supplied by publisher]

Impact of Frailty on Outcomes in Acute Type A Aortic Dissection.
CONCLUSIONS: Frailty, as defined using a 7-component frailty index, can serve as an independent predictor of the risk of late mortality in patients undergoing an ATAAD. Such frailty markers, all of which are easily assessed preoperatively, may provide valuable information for patient counseling and risk stratification before aortic surgery. Complete sternal-sparing HeartMate 3 implantation: a case series of ten consecutive patients.
CONCLUSIONS: Our early outcomes suggest that the CSS approach may reduce the incidence of right ventricular failure, bleeding, intubation time and ICU length of stay. Further studies are needed to fully elucidate the advantages of CSS LVAD implantation compared with median sternotomy. PMID: 30444989 [PubMed - as supplied by publisher]

The Society of Thoracic Surgeons Intermacs Database Annual Report: Evolving Indications, Outcomes, and Scientific Partnerships.
CONCLUSIONS: With the evolution of MCS, patient phenotype and outcomes are also changing over time. CF LVAD support is increasingly being used in the less ill patient phenotype and more patients are supported for destination therapy. Mean survival is now approaching 5 years, but adverse events, especially neurologic events, continue to have a detrimental impact on the success of CF LVAD support. Impact of Bleeding Revision on Outcomes after Left-Ventricular Assist Device Implantation.
CONCLUSIONS: A bleeding revision is associated with significantly reduced survival and substantially increased morbidity during LVAD support. Pre-, intra- and post-operative prophylactic strategies may help to prevent this life-threatening complication. PMID: 30851258 [PubMed - as supplied by publisher]

The Impact of Preoperative Clopidogrel on Outcomes Following Coronary Artery Bypass Grafting.
CONCLUSIONS: CABG can be safely performed in patients with recent Clopidogrel exposure with no increased risk of early or late mortality. However, CABG should be delayed for at least 5 days if clinically feasible to minimize transfusions and reoperation risk. In patients with more urgent indications, delaying CABG at least 3 days can mitigate but not eliminate the increased bleeding risk. PMID: 31039353 [PubMed - as supplied by publisher]

Transcatheter and Surgical Aortic Valve Replacement in Patients with Recent Acute Heart Failure.
CONCLUSIONS: Recent AHF is associated with increased risk of mortality and morbidity after SAVR and TAVR. These findings suggest that AS patients should be referred for invasive treatment before the development of clinically evident heart failure. PMID: 31288017 [PubMed - as supplied by publisher]

HVAD Cannula Position and Hemocompatibility-related Adverse Events.
CONCLUSIONS: HVAD cannula coronal angle was associated with reduced right ventricular function and HRAE. Prospective studies evaluating surgical techniques to ensure optimal device positioning and its effects on HRAEs are warranted. PMID: 32035046 [PubMed - as supplied by publisher]

National Outcomes of Surgical Embolectomy for Acute Pulmonary Embolism.
CONCLUSIONS: In this contemporary, real-world study, mortality occurred in 19.8% of patients undergoing surgical embolectomy for acute pulmonary embolism. This represents a significant improvement compared to traditional outcomes and supports the role of surgery in the multidisciplinary treatment of this high-risk condition. Impact of Hematologic Malignancies on Outcome of Cardiac Surgery.
CONCLUSIONS: Although late survival is reduced in patients with hematologic malignancies, early outcomes are generally similar to matched controls. Thus, operation should not be withheld in patients with a diagnosis of hematologic malignancy who would benefit from cardiac procedures. PMID: 32822668 [PubMed - as supplied by publisher]

The Society of Thoracic Surgeons Intermacs 2020 Annual Report.
Abstract The STS-Intermacs 2020 Annual Report reviews outcomes on 25,551 patients undergoing primary isolated continuous flow left ventricular assist device (LVAD) implantation between 2010-2019. In 2019, 3,198 primary LVADs were implanted, which is the highest annual volume in Registry history. Compared with the previous era (2010-2014), patients implanted in the most recent era (2015-2019) were more likely to be African American (26.8% vs. 22.9%, P<.0001) and more likely to be bridged to durable LVAD with temporary mechanical support devices (36.8% vs 26.0%, P<.0001). In 2019, 50% of patients were INTERMAC...

New oral anticoagulants versus warfarin in atrial fibrillation after early post-operative period in patients with bioprosthetic aortic valve
CONCLUSIONS: In real-word, NOACs use overcomes the indications provided by guidelines. This study evidenced that NOACs use in patients who developed AF after bioprosthetic AVR was more effective in prevention of thromboembolism and safe in reduction of major bleeding events compared to warfarin.PMID:33744222 | DOI:10.1016/j.athoracsur.2021.03.016

Ticagrelor or aspirin after coronary artery bypass in patients with chronic kidney disease
CONCLUSIONS: In patients with CKD and CABG, those who received ticagrelor had a higher incidence of MACCE but a similar incidence of major bleeding compared to those who received aspirin.PMID:33819474 | DOI:10.1016/j.athoracsur.2021.03.061
