Antibiotics in eosinophilic chronic rhinosinusitis: Rethinking maximal antimicrobial medical therapy
ConclusionsThe use of antibiotics without independent anti‐inflammatory properties have limited efficacy in patients with eCRS. The concept of maximal medical therapy using antibiotics must continue to evolve in the setting of new evidence for inflammatory subtypes of CRS. Level of Evidence2b. Laryngoscope, 2016

The prevalence of olfactory dysfunction in chronic rhinosinusitis
ConclusionsIn CRS populations, a significant percentage of patients experience olfactory dysfunction, and mean olfactory scores are within the dysosmic range.Laryngoscope, 2016

Prevalence of polyp recurrence after endoscopic sinus surgery for chronic rhinosinusitis with nasal polyposis
ConclusionsPolyp recurrence is common after ESS with control of polyps up to 18 months found in approximately 60% to 70% of patients. Investigation into both surgical and medical management strategies is warranted to improve upon the observed prevalence of recurrence. Level of Evidence2c. Laryngoscope, 2016

Rapid hemostasis in a sheep model using particles that propel thrombin and tranexamic acid
ConclusionsSPTT significantly reduced bleeding time in a sheep model of surgical sinus bleeding and successfully stopped bleeding following catastrophic carotid artery injury, with no adverse events observed. Level of EvidenceNA Laryngoscope, 2016

Balloon dilation of sinus ostia in the Department of Defense: Diagnoses, actual indications, and outcomes
ConclusionsBalloon dilation of sinus ostia has an expanding role in treating sinus disease. In the studied population, BCD is often utilized for alternate indications for which there is currently no evidence of efficacy. Future studies are needed to evaluate the efficacy of this technology in treating these alternate indications. Level of Evidence4. Laryngoscope, 2016

The genetics of the bitter taste receptor T2R38 in upper airway innate immunity and implications for chronic rhinosinusitis
ConclusionTaste receptors are emerging as critical components of early‐phase respiratory innate immunity, detecting molecules used by microbes to communicate and stimulating localized host defenses. Genetic polymorphisms are very common within the taste receptors, and recent linkage studies have demonstrated associations of taste receptor genetics with CRS. Lastly, ciliogenesis, which is often impacted in CRS, is critical for the functional expression of T2R38. Level of EvidenceN/A. Laryngoscope, 2016

Secreted P ‐glycoprotein is a noninvasive biomarker of chronic rhinosinusitis
ConclusionP‐gp secretion levels are significantly elevated in patients with CRS. High P‐gp secretion is associated with a higher incidence of CRSwNP and confers worse subjective and objective measures of disease severity. The presence of elevated P‐gp secretion may therefore represent a novel noninvasive biomarker of CRS and could be used to predict patients who may benefit from P‐gp inhibitory therapeutic strategies. Patterns of olfactory impairment reflect underlying disease etiology
ConclusionThis is the first study to comprehensively show that patterns of olfactory impairment reflect underlying disease etiology. We suggest that multicomponent olfactory testing should be performed, especially if there is diagnostic uncertainty. However, to clearly separate different patterns of olfactory loss to the various causes at an individual level, more work is needed. Level of Evidence4. Laryngoscope, 2016

Variable utilization patterns of computed tomography for rhinosinusitis in emergency departments
ConclusionsSeveral socioeconomic, demographic, and geographic factors are associated with sinus CT scan use in the ED for rhinosinusitis. This study has implications for CT imaging guidelines in the ED setting and demonstrates a need for further investigation of healthcare utilization disparities. Level of Evidence2C. General antibiotic exposure is associated with increased risk of developing chronic rhinosinusitis
ConclusionUse of antibiotics more than doubles the odds of developing CRSsNP and is associated with a worse QOL for at least 2 years following exposure. These findings expose an unrecognized and concerning consequence of general antibiotic use. Level of Evidence3b. Laryngoscope, 2016

Recurrence of sinonasal inverted papilloma following surgical approach: A meta ‐analysis
ConclusionsSurgical management of IP via an endoscopic approach reduces the risk of recurrence compared to an external approach. Although further data are needed, early‐ stage IP requires endoscopic or endoscopic‐assisted surgery to reduce the risk of tumor recurrence. Level of EvidenceNA Laryngoscope, 2016

Training model for control of an internal carotid artery injury during transsphenoidal surgery
ConclusionThis ICA injury model is portable; reproducible; and avoids ethical, biohazard, religious, and legal problems associated with cadaveric models. A synthetic ICA injury model for EEA allows recurring training that may improve the surgeon's ability to maintain endoscopic visualization, control catastrophic bleeding, decrease psychomotor stress, and develop effective team strategies to achieve hemostasis. Cost utility analysis of endoscopic sinus surgery for chronic rhinosinusitis with and without nasal polyposis
ConclusionThis study demonstrates the cost‐effectiveness of ESS compared to medical therapy alone for the management of CRS patients both with and without NP. The presence of nasal polyps was not found to affect the overall cost‐effectiveness of ESS. Level of Evidence2C. Laryngoscope, 2016
