Synchronous bilateral ovarian torsion followed by recurrent unilateral torsion in a patient undergoing fertility treatment: A case report and literature review
CONCLUSIONS: Clinicians should have a low-threshold to investigate and rule out OT in patients who present with lower abdominal pain, especially in those with additional risk factors for torsion. Patients with confirmed torsion can be successfully managed with detorsion of the ovaries. Further research is needed to determine the best management option for patients with recurrent torsion episodes.PMID:37734139 | DOI:10.1016/j.ejogrb.2023.08.015

Efficacy and safety of patient-controlled thoracic epidural analgesia alone versus patient-controlled intravenous analgesia with acetaminophen after laparoscopic distal gastrectomy for gastric cancer: a propensity score-matched analysis
CONCLUSIONS: PCIA + Ace can be safely applied without an increase in complications or deterioration in gastrointestinal function; moreover, PCIA + Ace use may provide better pain control than PCEA use in patients following LDG.PMID:37653160 | DOI:10.1007/s00464-023-10370-w

Watchful waiting for large primary nonparasitic splenic cysts
CONCLUSION: Asymptomatic patients managed nonoperatively for large NPSC did not become symptomatic or require intervention during the study period. This supports watchful waiting with serial radiologic and clinical monitoring for asymptomatic large NPSC.PMID:37500107 | DOI:10.1503/cjs.010322

Multimodal Pain Control Utilizing Buprenorphine for Robotic Assisted Laparoscopic Prostatectomy: A Quality Improvement Comparison to Conventional Opioid Management
CONCLUSIONS: Our study demonstrates buprenorphine's analgesic capabilities to maintain adequate pain control and patient satisfaction compared to conventional opioids during robotic assisted laparoscopic prostatectomy, while decreasing perioperative opioid use.PMID:37498667 | DOI:10.1097/UPJ.0000000000000436

Manifestation of acute appendicitis as known but paradox visceral side effect of ulcerative colitis anti-inflammatory therapy with januskinase-inhibitor Tofacitinib (Xeljanz ™)
CONCLUSION: Based on the paradoxon of an acute inflammatory disease (acute appendicitis) seen in the case of a patient undergoing immunosuppressive/anti-inflammatory treatment using a JAK-Inhibitor for ulcerative colitis, we consider this case worthy of publication although this side effect has previously been described in patients with rheumatoid arthritis. This might be the manifestation of i) an immunomodulatory effect that reduced or at least altered mucosal defense, including an increased risk of opportunistic infections, presenting as a specific visceral 'side effect' of the JAK-Inhibitor and/or as a consequence; ii)...

Laparoscopic approach in postpartum pyoperitoneum: A guide to early rehabilitation
Trop Doct. 2023 Jun 26:494755231185367. doi: 10.1177/00494755231185367. Online ahead of print.ABSTRACTSepsis is a leading cause of maternal mortality and morbidity worldwide. Pyoperitoneum is a grave and life-threatening manifestation of puerperal sepsis. Administration of broad-spectrum antibiotics and drainage of pus by laparotomy has long been the cornerstone of treatment for pyoperitoneum in a parturient. In this series of six cases, the successful laparoscopic management of postpartum pyoperitoneum is discussed. The latter offers the advantage of a magnified view of the surgical field, thorough lavage and drainage, an...
