Bypass and other modified reconstruction techniques for 'challenging' carotid cases: A comparison with conventional endarterectomy
CONCLUSIONS: Patients who underwent ICA revascularization with MCAR showed risks of perioperative death, major or minor stroke (<2%), reintervention rates and carotid restenosis rates that are comparable with PCEA or ECEA groups. Nevertheless, the MCAR group showed a significantly higher rate of global central neurological complications (considering together TIA, minor stroke and major stroke) than patients treated with standard CEA. MCAR techniques appear to be effective alternatives to standard CEAs, with an acceptable surgical risk. However, these should be performed mainly in selected cases, for example, in complex ...

Nationwide outcome analysis of primary carotid endarterectomy in symptomatic patients depending on closure technique and patch type
CONCLUSIONS: There was an increased risk of ipsilateral stroke <30days in patients operated with primary closure compared to eversion-CEA and patch angioplasty. There was no difference between primary closure, different patch types or eversion after the perioperative phase.PMID:36603661 | DOI:10.1016/j.ejvs.2022.12.033

Isolated hand weakness due to brain lesion
Hand Surg Rehabil. 2022 Aug 12:S2468-1229(22)00172-4. doi: 10.1016/j.hansur.2022.08.003. Online ahead of print.ABSTRACTWhen hand weakness is linked to a brain lesion, there is risk of misdiagnosis. A case of hand weakness related to stroke is presented. The electromyography report indicated a segmental lesion of the median nerve at the wrist. Thus, the patient was initially diagnosed with carpal tunnel syndrome, and was referred to the orthopedic department for release surgery. Because the symptom was immediate and did not include hand numbness, the orthopedic surgeon ruled out the previous diagnosis and ordered a head CT ...

Outcomes and role of shunting during carotid endarterectomy for symptomatic patients
In this retrospective study of 4,652 patients with symptomatic carotid stenosis undergoing CEA and patch angioplasty, shunt placement was associated with two-fold increased risk of cranial nerve injury without reduction in postoperative stroke risk, irrespective of symptom severity.

Peri-procedural variables and outcomes of long-period hemodynamic instability after carotid artery angioplasty and stenting
CONCLUSIONS: CAS-induced HI occurs in a considerable percentage while several peri-procedural variables are determined as independent predictors to develop long-period HI. Patients with prolonged HI are associated with increased risk of neurologic events and thus standardized intervention as well as management of long-period HI are of critical importance during clinical process.PMID:35466838 | DOI:10.1177/17085381221091369

An alternative reduced dose regimen of ticagrelor for neuroendovascular patients
CONCLUSIONS: A lower dose of ticagrelor (45 mg twice daily) appears to be safe and effective in this small cohort of patients who are resistant to clopidogrel per P2Y12 testing and who have increased risk of ischemic or hemorrhagic strokes due to neurovascular pathologies and implants. Further randomized studies are required to confirm these findings.PMID:35466828 | DOI:10.1177/17085381221092858

Carotid Endarterectomy with Patch Angioplasty versus Primary Closure in Patients with Symptomatic and Significant Stenosis: A Systematic Review with Meta-Analyses and Trial Sequential Analysis of Randomised Clinical Trials
Introduction: Patch angioplasty in conventional carotid endarterectomy (CEA) is suggested to reduce the risk of restenosis and recurrent ipsilateral stroke, compared to primary closure. A systematic review of randomised clinical trials is needed to compare the outcomes (benefits and harms) of both techniques.

Immediate and Long Term Results of a Randomised Single Centre Study of Surgical Treatment of Asymptomatic Stenosis of the Internal Carotid Arteries
Introduction: Cerebrovascular accident (CVA) is one of the main causes of disability and mortality in many developed countries of the world. The results of numerous studies have confirmed the positive effect of carotid endarterectomy (CEA) in the prevention of stroke development in both symptomatic and asymptomatic patients. Balloon angioplasty (BA) and carotid artery stenting (CAS) have evolved, in part, because of the need for alternative, less invasive treatments for patients considered poor candidates for surgery based on anatomical or clinical parameters.
