Open thoracic and thoracoabdominal aortic repair vs. f/bTEVAR - complementary or competitive?
In conclusion, in our opinion OR is a service that is still necessary for dedicated aortic centres, but will most likely become more frequent again in the years to come.PMID:34666359 | DOI:10.1055/a-1562-2770

Association Between Hospital Volume and Failure to Rescue After Open or Endovascular Repair of Intact Abdominal Aortic Aneurysms in the VASCUNET and International Consortium of Vascular Registries
Objective: To investigate the association between hospital volume and failure to rescue (FtR), after open repair (OAR), and endovascular repair (EVAR) of intact abdominal aortic aneurysms (AAA) among centers participating in the VASCUNET and International Consortium of Vascular Registries. Summary of Background Data: FtR (ie, in-hospital death following major complications) is a composite end-point representing the inability to treat complications effectively and prevent death. Methods: Using data from 8 vascular registries, complication and mortality rates after intact AAA repair were examined (n = 60,27...

Thoracic Endovascular Repair for Aortic Arch Pathologies with Surgeon Modified Fenestrated Stent Grafts: A Multicentre Retrospective Study
CONCLUSION: TEVAR with surgeon modified fenestrated stent grafts for the treatment of aortic arch pathologies provides acceptable outcomes. Further follow up is required to confirm the benefits of this approach.PMID:34629276 | DOI:10.1016/j.ejvs.2021.07.017

Risk factors Associated with Long Term Mortality and Complications Following Thoracoabdominal Aortic Aneurysm Repair
To determine the risk factors associated with late mortality or complications (Thoracoabdominal aortic Aneurysm Life-altering Events (TALE): composite of mortality, permanent paraplegia, permanent dialysis, and stroke) in patients undergoing endovascular or open thoracoabdominal aortic aneurysm (TAAA) repair.

Lower Hospital and Operating Room Charges for Patients Undergoing Carotid Repair With Carotid Endarterectomy Versus Transcervical Carotid Artery Revascularization
Transcervical carotid artery revascularization (TCAR) has become an increasingly popular method for the treatment of carotid artery stenosis, especially because trial data have demonstrated a stroke risk comparable to that for carotid endarterectomy (CEA). Recent data have suggested that TCAR might be cost-effective; however, the costs associated with using this new technology have not been thoroughly evaluated.

The Potential Benefits and Costs of an Intensified Approach to Low Density Lipoprotein Cholesterol Lowering in People with Abdominal Aortic Aneurysm
CONCLUSION: People with a small asymptomatic AAA are at high risk of MVE and PVE. This study provides evidence of the possible benefits and allowable expense for a cost effective intensive LDL-C lowering programme in this population.PMID:34507892 | DOI:10.1016/j.ejvs.2021.06.031

Composite Dialysis, Paralysis, Stroke, or Mortality After Endovascular Aortic Interventions in the Society for Vascular Surgery Vascular Quality Initiative
Thoracoabdominal aortic aneurysm life-altering events, which include a combination of permanent dialysis, permanent spinal cord ischemia, stroke, and/or death, have devastating effects after complex endovascular repair. However, the occurrence of these life-altering events after endovascular abdominal aortic aneurysm repair (EVAR) and thoracic endovascular aortic repair (TEVAR) has not been studied. Therefore, we examined the effects of procedural and anatomic characteristics on a composite outcome of postoperative dialysis, stroke, paralysis, and/or mortality after any endovascular aortic repair.

Transposition of Anomalous Left Vertebral to Carotid Artery During the Management of Aortic Disease
The left subclavian artery is frequently revascularized by transposition or bypass when zone 2 thoracic endovascular aortic repair is planned, because decreased rates of spinal cord ischemia (SCI) have been reported with this strategy. An anomalous left vertebral artery (aLVA) originating directly from the aortic arch is the second most common arch variant after the bovine trunk, with a reported incidence of 4% to 8%. In addition, 6% to 10% of vertebral arteries will terminate in a posterior inferior cerebellar artery, increasing the risk of stroke if not revascularized.

Long-term Use of Preoperative Beta-Blockers Is Associated With Reduced Postoperative Stroke After Thoracic Endovascular Aneurysm Repair
Thoracic endovascular aortic repair (TEVAR) has been established as the treatment of choice for a range of thoracic aortic pathologies. Beta-blockers represent a possible perioperative treatment option to improve TEVAR outcomes due to their role in management of aortic aneurysm, dissection, and Marfan syndrome. We investigated the effect of preoperative beta-blocker usage on outcomes of TEVAR using the Vascular Quality Initiative database.
